Несмотря на улучшение качества жизни после КШ, у ряда больных развиваются нейропсихологические когнитивные нарушения, которые после КШ встречаются в 12–79% случаев. В связи с этим проблема неврологических осложнений в кардиохирургии продолжает оставаться актуальной. У 30–35% пациентов нарушения когнитивных функций сохраняются более года после операции. В представленном обзоре рассматриваются вопросы этиологии, патогенеза и распространенности когнитивных нарушений у больных с ишемической болезнью сердца после операции КШ. Факторами, способствующими развитию стойкого когнитивного дефицита, являются пожилой возраст, выраженное снижение когнитивных функций на предоперационном этапе. В настоящее время принято считать основными факторами риска неврологических осложнений КШ возраст, сопутствующие заболевания (артериальная гипертензия, сахарный диабет, сосудистые заболевания, заболевания легких), сердечную недостаточность, а также злоупотребление алкоголем, курение и предоперационное когнитивное снижение.
