Новые аспекты патогенеза диабетического кетоацидоза: интеграция экспериментальных и клинических данных

Аннотация

В обзоре освещаются современные механизмы патогенеза диабетического кетоацидоза (ДКА) на основе анализа экспериментальных и клинических данных. Описываются молекулярные пути, включающие активацию инфламмасомы NLRP3 (nucleotide-binding domain, leucine-rich repeat and pyrin domain containing protein 3) с последующим высвобождением провоспалительных цитокинов интерлейкина-1β (IL-1β) и интерлейкина-18 (IL-18), а также инициированием пироптоза β-клеток островков Лангерганса. Рассматривается дисфункция митохондрий и увеличение оксидативного стресса, способствующие повреждению клеток и усилению воспалительной реакции.

Проведен анализ дисрегуляции α-клеток поджелудочной железы с повышенной секрецией глюкагона, стимулирующей глюконеогенез и кетогенез. Обсуждена роль почек в поддержании уровня кетоновых тел, включая фильтрацию, реабсорбцию и взаимодействие с липидными сигнальными путями, влияющими на инсулинорезистентность и воспаление. Описаны центральные нейрогуморальные механизмы, вовлекающие гипоталамо-гипофизарную ось и симпатическую нервную систему, а также особенности ДКА, возникающего при терапии ингибиторами натрий-глюкозного котранспортера 2-го типа (SGLT2).

Подчеркивается значение осмотического стресса и его взаимодействие с контринсулярными гормонами. Представляется интегративная модель, объединяющая воспалительные, метаболические, эндокринные и почечные компоненты, что обеспечивает более глубокое понимание клинических проявлений ДКА и открывает перспективы для разработки новых диагностических и терапевтических стратегий.
