EDITORIAL
As a result of many years experiments by studying of blood system function under radiation and cytostatic myelosuppressions it has been shown, that observing changes and mechanisms, underlying in their basis, are unspecific and of the same type. However the specific reaction of blood system depends on the nature of acting irritant. The hemopoiesis recovery dynamics, together with direct suppressed effect of toxic agents on hemopoietic cells, in many respects, is determined by character of regulatory systems, and first of all separate elements, forming hemopoiesis – inducing microenvironment.
ASSEMBLY LECTION
New phenomena in structural and functional organization of the human genome, which turned out to be unexpected for the postulates of classical genetics (liberties of genome) are presented. Modern ideas on forms of inherited variability are given. Participation of new genomic phenomena in pathogenetics and in revealing of new form of human pathology (prion diseases, mitochondrial diseases, trinucleotide repeats expansion, genomic imprinting) is noted. The significance of synthropy conception for the investigation of genetics of common diseases is substantiated. The thesis on the unity of three parts of medicine (social, genomic, and individualized) is put forward.
SUB-FACULTY SCIENTIFIC MEDICAL SCHOOLS
Alexander S. Dogel (1852—1922) is salient Russian scientist, one of Neurohistology’s founders, the first manager of Histology’s department of Tomsk University (1888—1895), professor of medicine, Associate Member of Russian Sciences Academy, the founder of «Russian Archive of Anatomy, Histology and Embryology» journal, member of Committee of Nobel’s premium’s award. His scientific works in region of Neurohistology and sense’s organs are fundamental. Some kinds of retina neurons and vegetal ganglions are named after Dogel in the whole world.
EXPERIMENTAL AND CLINICAL INVESTIGATIONS
The research of lymphoid organs of male rats of Vistar line wich were subjected to vibration with freguency of 32 Hz during 30 days has been conducted. Cytological shifts of the basic structural components of the investigated organs during the vibration period, postcontact rehabilitation period (30, 60 days) and under essencial phospholipides correction were estimated by the methods of light and electron microscopy and morphometry.
The analysis of the obtained data demonstrated that the important infringement of lymphoid organs structures takes place. The dynamic study of morphofunctional and ultrastructural changes, cytoarchitectony of parenchymatous stromal compartment gives evidence of the development of degenerate-distrophic processes with mainly involutional character. Pharmacological correction resulted in positive dynamic of cytological shifs and helped to rehabilitation the natural cell phenotype.
The aim of this work is to gind ont influence of сarbohydrate сomponent of membrane glycoprotein receptors on the change of its functional activity at patients with acute coronari syndromes and its combination with diabetes mellitus type 2. 69 sich men and 20 nearly healthy men were examined. Among the patients 32 men sugger from acute coronari syndromes, 17 men — from combination with diabetes mellitus type 2 and 20 men — from diabetes mellitus type 2 only. Neutrophil aggregation induced by method by Gabbasov and another autors (1989). Wheat Germ Agglutinin (WGA), Concanavalin A (Con A), Phitogemagglutinin (PHA).
At patients with acute coronari syndromes aggregation of neutrophile isincreaced under affect of N-acetyl-D-glucosamin and acidi syalici and at patients with diabetes mellitus type 2 — under affect of mannose and bD-galactose. In the case of the combination of acute coronari syndromes and diabetes mellitus type 2 the rise of aggregation is connected not only with increase of mannose and bD-galactose but also with increase of N-acetyl-D-glucosamin and acidi syalici. Neutrophile' carbohydrate structure is restored under affect of defenite therapy.
The aim of the research is to determine the character and development of a alterations in photoreceptors, caused by high-intensity light on the early stage of diabetes mellitus. The methods of light and electron microscopy were used.
Experiments were obtained from 80 adult white male rats. The rats of group 1 (n = 20) received 1 injection of alloxan (15 mg per 100 g of weight) in order to model diabetes mellitus. The rats of group 2 (n = 20) were exposed for light of luminecent LB-40 lamps (6000 lx) for 6 hours. The rats of group 3 (n = 20) were exposed to the light with the same parameters 1 month after alloxan injection. The last 20 rats made a control group 4. The rats groups 2 and 3 were sacrifised by decapitation 1, 7, 14, 30 days after the light exposure, the rats of group 1 on the 4, 5, 6, 8 week after the alloxan injection.
Light exposure combined with diabetes causes destruction pigment epithelium inner and outer processed of photoreceptors followed by pyknosis of nucleus and replacement by vacial glia. On the 7th day after the exposure local disappearance of according retinal layers was obsevred. In these local sites of the most intensive destruction in group 3 there is 1,6 times more cells with caryopyknosis from in according sites of group 2 (just light exposure) rats.
Thus, alloxan diabetes on the early stage of its development (1 month) intensifies light damage of retinal photoreceptors. Probably, increase of oxydative processes and antioxidative systems break down play an important role in this synergetic effect.
At pregnant women with gestosis the increasing of concentration in blood serum of soluble molecules of adherence sVCAM-1 and sICAM-1 is discovered. The same changes are revealed at pregnant with bronchial asthma, the period of gestosis being 18 weeks. With it there is a positive correlation between the substance of soluble molecules of adherence and the concentration of cytokines of atopic inflammation in blood serum.
THE RESULTS OF YOUNG SCIENTISTS’ AND STUDENTS’ INVESTIGATIONS
From the overground part of Atragene speciosa Weinm. family Ranunculaceae, we obtained a fraction possessing a marked nootropic effect from which 3,4-dihydroxyphenylethanol-2, its 2-O-β-D-glucopyranoside, and a sum of glycosides of dihydroxybutane acid were isolated. In addition to the foregoing substances, in the pharmacologically active fraction we revealed the presence of triterpene glycosides (whose sapogenins are hederagenine and oleanolic acid), aminoacids and microelements.
For the frist time the obtained data about the biologically active substances and their pharmacological activity give us the possibility of development of a new phytopreparation with the nootropic effect on the basis of Atragene speciose Weinm.
SCIENTIFIC AND STUDYING PROCESS: METHODIC SEMINAR
The medicine is weakly formalized domain, and preliminary study of knowledge structure is necessary for computerlearning programs development. Semantic network can be offered as an universal knowledge model for higher medical education. The links between nodes reflect their semantic distance. Graph theory is an adequate tool for knowledge structure investigation, and the results of one prove knowledge models for certain domains and optimal navigation through the program. The last depends on personal features of students, too.
So, the basic principles for computer learning programs development in higher medical education are knowledge structure modeling and individual learning navigation.
In the article the question of regulation of the relations between the employer and employers is discussed. With the aim to define these relations there was established, inner Thesis about the office inventions in Siberian State Medical University.
THE PAGE OF PRACTICAL DOCTOR
The results of inspection and surgical treatment 38 patients with opisthorchiatic cysts of a liver are investigated. Preoperative diagnostics is based on detection opisthorchiasis and specific ultrasonic attributes of these cysts, opisthorchiatic lesion of hepatobiliary system. The complicated current it were marked at 21% of the patients. Carry out 40 operations, including the resections of a liver. Mortality rate was 5,3%. Developed surgical tactics and technique allows to warn specific complications not reducing radicalism of operations.
The long-term opisthorchiasis invasion leads to serious complications followed by surgery. Between 1170 patients with O.felineus, undergone an operation at the Tomsk hepatologic center, carcinoma of the liver, bile ducts and pancreas was revealed in 13% of cases. Clinical manifestations of carcinoma are masked by signs of opisthorchiasis for a long time, that leads to the process of a disease of stage III—IV. Ultrasonography and endoscopic methods of examination such fibrogastroscopy, pancreatography, laparoscopy seem to be of great importance in diagnosis. All operations are, mainly, of palliative (57,4%) or explorative (13%) character. Radical operations were made in 10,6% of cases only, laparotomy was of no use in 19% of patients. It is recommended to make a prophylactic medical examination of patients with opisthorchiasis including obligatory ultrasound and endoscopic methods once a year at least, an adequate timely dehelminthization.
Nervous system syphilis — neurosyphilis (NS) belongs to rather rare diseases. According to the different authors data available it comes to about 1% of the nervous system organic disturbances and develops by 5—10% syphilous patients not having been treated in the acute stage of the pathological process. The syphilitic disturbance of the nervous system is a chronic progressive disease caused by the pale spirochaeta. The nervous system disturbance occurs basically in two ways: secondary one, because of brain shells, vessels, gummatous manifestations involving in the pathologic process; or primary one, when the causative agent affects directly the brain substance. In the first case syphilis is called rnesodermic or early one; in the second case — parenchymatous or late, primary one.
In the article the analysis of two clinical cases of mesenchyme neurosyphilis is carried out: latent neurosyphilis and syphilitic meningomyelitis. Neurosyphilis classification is applied to clinical manifestatons of syphilitic arachnoiditis and vasculitis are described. The methods of specific and nonspecific therapy of mesenchyme syphilis are stated in detail.
Patients with an appendicular peritonitis were studied with the disfunctions of a hemostasis and modular conditions of a blood. The experiments were carryed-out with the help of an analyzer of the reological properties ARP-01 «Mednord». 22 children had been examined. The scheme of the developing hypocoagualation was developed. The positive effect of combination of controlled gipocoagulation and antibiotic therapy is marked with the help of cell-like mass inkubation method.
Alongside with economic problems of development of North the important place is borrowed by a problem of guarding of health of the population. The population of Boreal districts of Siberia is subject to effect of a lot of the unfavorable factors both connatural (climatic conditions), and anthropogenic (ecological trouble) character. A corollary it is the high level of a case rate, violation on the part of cardiovascular, nervous and other systems of an organism.
The inhabitants of boreal districts, workers of oil-extracting branch are bunch of the boosted risk on creation of a pathology of organs of a circulation and endocrine system. The data quota needs a regular control of a state of health and opportune rendering of a medical care in amplate volume. The optimal form of organization of a medical care to the population of Boreal districts is the direction of exit medical crews in items of their residing. Such form of work is economically favourable and at the least expenditures allows to reach appreciable social and medico-sanitary effect.
FROM SCIENCE TO PRACTICE
According to the fact, 30% of women with mammary cancer are younger than 40 years old. It is the age when the danger of this disease is the highest. The aim of the work is the identify the diagnostic effectiveness of ultrasound research method in the first level of mammary cancer developed in fibro adenoma or in cyst, together with the nonmalignant proliferative diseases of mammary. Ultrasound research appeared to be a very effective method of diagnostics of cysts and fibroadenoma together with the expressed diffuse pathology of mammary. The differentiation line of diagnostic criteries at fibroadenoma and carcinoma if defined and the diagnostic algorithm is worked out. The necessity to use sonography as the main method of examining women (younger than 50 years old) at mammary disease is discussed.
CHRONICLE OF SIBERIAN MEDICINE
On the base of archive documents the comparative characteristics of the situation in health service in Tomsk of the beginning of XX century and of modern situation in health service in Tomsk is given.
The biography of the famous scientist of the Siberian medicine, the first man began studying the history of the Siberian medicine, professor N.P. Fedotov is presented.
DISCUSSIONS
The authors offer the hypothesis of reception of a geomagnetic field based on the special properties of a mammal’s skin. It assumes that the ordered arrangement of hair follicles (Foit lines) can serve the pseudo-solenoid, in which under the action of external fields the currents starting the neuro-humoral mechanism of answer-back reactions are induced. According to this hypothesis, the magnitude of the generated afferent neuro-humoral signals depends on animal orientation concerning a vector of a geomagnetic field. There of there will be a larger or smaller degree of activation of hypothallamus-hypophysis-adrenal system (HHAS) which defines the adaptive status of an animal and, accordingly, adequacy of response to the set of external factors corresponds.

In the article the strategy of the library- informational provision of the education for the satisfaction of the workers and the students of the university in the changing conditions of informing and automation is presented.
THE FACE OF THE ISSUE
The author of this article describes some directions in the development of the Tomsk Scientific School of Pathophysiologists (1939–2001) by the description of the life of V.S. Lavrova, the teacher and the researcher of Pathophysiology Department. In the work her private memories, the memories of her students and colleagues, the archive materials are used. Some of them are published for the first time.
