EXPERIMENTAL AND CLINICAL INVESTIGATIONS
Peculiarities of pathogene sis of Helicobacter pylori (HP)-associated gastric ulcer (GU) in Khakas se s are investigated. It is found that, in con - trast to Caucasian s, the activation of the Т-cell and humoral parts of the immune response in Mongoloids with GU is accompanied by the high con - tent of CD4 +- and CD20 +-lympho cytes in the muscou s coat of stomach. Changes revealed in the peripheral blood in GU patients were character - ized by a decrease in the magnitude of CD4 +- and CD8 +-lympho cyte subpopulations compared to patients with chronic gastritis. The detected pos - itive correlation between the АА-251 genotype of interleukin-8 and the risk of GU development in Khakssses indicated in favor of the existence of immuno-genetic regularities of GU development. The reasonability of determining population risk and protective genotypes of development of HРassociated GU in Khakasses is discussed.
In the experiment with Wistar lab rats, the structure, physical-chemical properties, the strength characteristics of the bone tissue under the joint action of natural zeolites and sodium fluoride are investigated. Morphological studies were carried out with the aid of the atomic-force and scanning electron microscopy. Physical-chemical studies were carried out with the aid of the Raman spectroscopy and the atomic absorption spectrophotometry. Micromechnic tests were conducted by the microindentation method with the use of a micro hardness meter. It was shown that natural zeolites used as food additives and acting together with fluorine imposes an extreme impact on the bone tissue due to hypermineralization effect on membranes of bone canalicules with the secondary disorder of trophics of the bone matrix and the worsening of bone hardness characteristics. These circumstances cast doubt on promises of using zeolite and fluorine-containing medical drugs in osteoporosis correction technologies.
In the experiment carried out on 150 Vistar line male rats after a thyreodectomy a thyroid gland tissue was autotranspalted into big omentum to prevent postoperative hypothyroidism. Three-month study and comparative analysis of morphology and functional state of the transplanted tissue showed high levels of thyroid hormone reactivation and transplant viability owing to the use of the cellular suspension of autologous pluripotent mesenchymal stromal cells.
А cytological technique to determine the character of pleura pathology with malignant tumors among 297 women was used. The features of malignant cells of tumors of reproductive system in pleurisies with breast carcinoma (119), ovarian (73) and endometrial carcinoma (4) were studied. A light microscopy, also immunocytochemical technique with a set of 2—14 antibodies in difficult cases for diagnostics was used. With the dissemination of breast carcinoma on pleura, the specific pathogonomic characters in 78.2 % of cases are found. With ovarian carcinoma according to the certain cytological features also taking into consideration immunocytological characters in 34, 2% of observations, the site of primary tumor was exactly revealed. Indirect characters were noticed in 61, 6% of cases. With endometrial carcinoma, the special specific characters were not observed. Immunocytochemical technique can play a critical role in a differential diagnose of canceromatous pleurisies.
The culture of stromal stem cells was incubated under constant and dynamic conditions. The incubation period was lasting 48 h. Intracellular enzyme activity of stromal stem cells were carried out with cytochemical methods. Alkaline phosphatase activity was increased under dynamic conditions, differentiation of stromal stem cells being accelerated comparatively steady-state conditions.
Clinical-functional peculiarities of bronchial asthma combined with the hypertonic disease have been revealed. It has been found that the combined pathology occurs more often in older age and proceeds with more pronounced clinical symptoms. It is characterized by the disturbed lipid exchange, diastolic disfunction, changes in the vegetative regulation of the cardiac activity. These symptoms should be taken into account in the plan of treatment.
In order to find effective sorbents of herbal origin, we’ve studied adsorption properties of aerial dry raw materials of 31 marsh-water, medical perspective, plant species. It was found that 8 of those 31 exercises adsorption activity (Shpagnum species).
Clinical-psychopathological examination has been carried out as well as quality of life has been studied in female patients with dysmenorrea in period of puberty and at early reproductive age. It has been revealed that mental disturbances in studied nosology were represented by their high prevalence. In girls-adolescents with this pathology neurotic reactions were found more frequently, in women with dysmenorrea at early reproductive age persistent neurotic states predominated. It has been shown that formation and dynamic of mental disorders were determined by somatogenic, psychogenic and social factors.
Results of annual examinations of peripheral blood and single examinations of bone marrow after 11, 14, 15, and 22 years of the chronic exposure to ionizing radiation in total doses of 24.25, 103.96, 111.84, and 74.09 mSv, respectively, are presented along with the results on the morphology of microvasculature vessels in skin tissue samples of two men and two women working at a nuclear reactor. Changes in the content of hemoglobin, total number of leucocytes, population of erythrocytes, neutrophils, reticulocytes, and in the composition of maturating and mature myeloid and erythroid cells accompanied by an increase составе in the number of cells of the lymphoid and monocytic pool as the intensity of inflammatory changes in microvasculature vessels increases from productive panvasculitis to panvasculitis with sclerosis of vessel wall were noticed. The presented actual material indicates that damages in the blood system can be connected with the intensity of pathological changes in microvasculature vessels, which represent one of the local components of hemopoiesis-inducing microenvironment responsible for hemopoiesis regulation.
The infectious complications of injuries and wounds are the actual problem of surgery. One of the most important tasks for today is the investigation of new remedies to prevent from the infectious process and to accelerate purifying processes from necrotic tissues in the wound and to intensify the wound regeneration. In this connection, to guarantee sorbing, antimicrobic, necrolitic and anesthetic protective actions is necessary to develop the first stage wound process bandage. As a result of the doctors’ and scientists’ team-work investigation a new dressing model for treating exuding wounds has been made. In the experiment this adsorbing bandage with the sorption level based on graphite particles (thermo broadened form of fluorinated graphite intercalated compound) has shown the high wound healing and drainage activity in treating associated gunshot bone-arterial injuries of extremities.
Twenty children aged 3—6 years with Epstein—Barr-virus-induced infectious mononucleosis (IM) were examined. The present study is aimed at investigating the functional activity of cytotoxic lymphocytes in children with IM during clinical and hematological manifestation of the disease and early reconvalescence phase. For analyzing the subpopulation composition of peripheral blood lymphocytes in children with IM the CD-receptors on cell surfaces (CD8, CD16) were defined; for estimating apoptosis the amount of CD95-, CD95L- and annexin V-positive lymph cells (immunofluorescence method) was assessed. For determination of TNFα concentration in cultural supernatants the solid-phase immunoenzyme method was applied. It was defined that clinical and hematological manifestation of IM is accompanied by increase in the amount of CD8-, CD16-lymphocytes in blood and activation of Fas-dependent apoptosis of lymphocytes that is preserved in early reconvalescence period. Here limitation of lymphocytic cytotoxicity results from a decrease in TNFα secretion.
THE RESULTS OF YOUNG SCIENTISTS’ AND STUDENTS’ INVESTIGATIONS
The influence of hepatoprotector phospholipid nature Essentiale and Essenciale combination with succinic acid on the metabolism of liver injury caused by Paracetamol in rats has been investigated. As a result of therapy was decreased Paracetamol hepatotoxicity — in the blood reduced the aminotransferases, alkaline phosphatase activity, bilirubin content, in liver homogenates decreased production of diene conjugates, Schiff bases and malondialdehyde. Essentiale improved the oxidative phosphorylation conjugacy without influence on liver mitochondria respiratory activity kinetic parameters. Combined application of Essentiale and succinic acid normalized the liver mitochondria respiratory function.
At multicenter prospective observational study conducted in 19 centers in Russia under common protocol (n = 543) was demonstrated that in the context of real clinical practice the use of a step-up treatment regime and long-term dosing in a stable volume leads to control achievement in 73,3 and 69,4% cases respectively.
The polymorphism of the gene of inducible NO-synthase (iNOS) was studied in 65 children with bronchial asthma (BA). The goal was to determine the probability to achieve the BA control in children against the background of inhalation glucocorticosteroid (IGCS) therapy depending on the polymorphism of the iNOS gene. The genotype of polymorphism of the iNOS gene was studied by the method of polymerase chain reaction. It has been found that the achievement of BA control in children against the background of the IGCS treatment is associated with the carriage of homozygote of «wild» allele G of polymorphism G954C of the iNOS gene. The absence of BA control is related to the presence of pathological allele . The result obtained can favor the pre С - diction of the uncontrollable BA course in children.
SCIENTIFIC AND STUDYING PROCESS: METHODIC SEMINAR
In history study, as a rule, happens to give the estimation to activity that or other scientist, quite often integer of the group, defining their role in formation and development of medicine of the region in determined chronical frame. However hitherto no united opinion for importances «scientific school». In work is made attempt of the analysis existing opinions on this subject, their collations and comparisons for the reason separations the most exhausting.

The clinical examination of 198 pregnant women after the health promotion program and their infants has been carried out. The content of stress hormones and the intensity of processes of lipid peroxidation have been studied, and adaptation abilities have been assessed. The use of the proposed rehabilitation program for pregnant women at early gestational ages favors the reduction of occurrence of feto-maternal disease and the increase of the adaptation reserve of women. Infants, whose mothers received the health promotional program, 3.5 times rarely had signs of the perinatal affection of the central nervous system.
On the grounds of ultrasonography of 275 patients with suspected Acute Appendicitis (AA) the diagnosis was confirmed in 63 (22,9%) cases, it was true-positive results; in 3 (1,1%) cases of non-confirmed AA ultrasound findings were regarded as false-positive results. True-negative results of examination were obtained in 194 cases (70,5%) and false-negative results — in 15 cases (5,5%). The presence of rarevascularity of interior wall and clear visualization of vessels in mesentery of vermiform appendix is typical for congestive appendicitis. When phlegmonous appendicitis there are numerous vascular branches in interior wall blood stream of appendix: they make a picture of color «crown» in horizontal section and color «stripe» in longitudinal section. Gangrenous appendicitis is attached to few color spots in those parts of appendix wall which are not destroyed yet. Vascular pattern in adjacent intestinal loops predominates over vascular pattern of vermiform appendix. In case of empyema vermiform appendix vascular pattern in the wall is detected only in mesentery area. Vascular pattern of appendix is not detected in cases of appendicular infiltrate and periappendicular abscess unlike vascular pattern of adjacent intestinal loops which is rather intensified. The sensitivity of ultrasonography in AA detection was 80,7%, specificity — 98,4%, and accuracy — 93,4%. High index of diagnostic effectiveness of the method was obtained because of adoption of combining B-mode with both Colour Doppler and Power Doppler methods.
FROM SCIENCE TO PRACTICE
The influence of the chronopharmacological scheme of treatment with the use of exogenous Melatonin in the form of the Melaksen drug on the immunity status of psoriasis patients has been studied. The study of the immunity status consisted in determination of the main Manchini classes of immunoglobulins A, M, G, the level of circulating immune complexes by the method of selective precipitation in 3% polyethylene glycol. In addition, we determined the subpopulation of lymphocytes carrying CD3, CD4, CD8, CD16, CD 25, and CD72 antigens by the immunofluorescent method. In has been found that Melatonin in psoriasis patients exerts the immunomodulatory action on the immune system at the combined treatment (due to an increase in the number of -lymphocytes with cytotoxic-suppressive properties at obvious signs of immune disbalance in the form of the increased ratio of Т CD4+ and CD8+).
In this article clinical indexes of the dynamics of mineral bone density according to both densitometry and marker of resorption as well as bone formation and steroid blood hormons in patients in postovariectomy are analysed. These patients state was complicated by cancer of the corpus and colli uterus after etiopathogenetic treatment including total husterectomy, radiation therapy and polychemotherapy. According the results of the comparative analysis in patients with postvariectomic syndrome developed after surgical treatment for benign internal genitalia diseases we may make the conclusion on pathogenetic role of postovariectomic syndrome in the development of bone disturbances in female patients which underwent total hysterectomy and special therapy.
ECONOMICS AND HEALTH CARE MANAGMENT
The opinion of doctors and patients on the suitability and forms of business activity in medicine is studied. For most people, free medical aid has not clear boundaries, and this fact significantly reduces the motivation to payment for medical services, that is, restrains the demand. The businessmen’s mentality is not formed as well: in the medical community, a medical service is not realized as a good, and the medical activity is not understood as an activity on production and marketing of medical services, the «kept» psychology dominates. The paying capacity of people should not be considered as a key factor restraining the development of medical business. The more significant problem is that the current medical system is not ready and capable to sell medical services to people having the considerably increased level of consumer demand.
