Abstract

Hypertensive intracerebral hemorrhage - a common form of cardiovascular disease, characterized by high mortality and disability. One area in improving patient care remains the development of new minimally invasive surgical methods and devices. We describe a new method and device (funnel cannula), which after the special markings to the use of CT punktion entered into the cavity, followed by the evacuation of hematoma. In each case, depending on the depth of hematoma and its volume is cannula certain size. The device allows for a more radical removal of hematoma without significant damage to brain tissue.

Abstract

There are psychologic status of patients having multipled sclerosis research results. 30 patients were involved in the research, among them there were 18 (60%) female and 12 (40%) male ones at the age between 20 and 60. Patient examination was realized by means of the Spielberger, Maudsley and Zung methods. As a result of research there are patient's psychological state changes. 66% of patients have these changes in the form of high-grade uneasiness, 50% — in the form of rigidity and introversion, 83% have depressing disorders in the form of slight depression and subdepression.

These findings expand our conception about psychological mechanisms of dissociated personality patients with multipled sclerosis have, and need various multimodal approaches for their psychological correction.

Abstract

Hypoxia and its related oxidate stress are the direct cause of the neural damage due to chronic cerebral ischemia. Resistance to hypoxia and oxidate stress is the main marker of the human and brain adaptation to chronic ischemia.

Hypoxia resistance and the condition of the glutation antioxidant system in the difference stages of chronic cerebral ischemia was investigated in this study. 310 patients from 35 to 55 years with the first (100 persons), second (105 persons) and third stages (105 persons)) of chronic cerebrovasculare insufficiency were examened.

Decreasing of the hypoxia resistance and resistance to oxidate stress at chronic cerebral ischemia was shown. The average figure of hypoxia resistance and functional activity of the glutation system at the different stages of the chronic cerebral ischemia shows the course of the general adaptive syndrome.

Abstract

Here we present the results of our study of the influence of clinical and social parameters as well as co-morbidities (as anxiety, depression, sleep disturbances) on the health related quality of life of 95patients with the most often met primary cephalgias — tension type headache. We used MOS-SF36 (Medical Outcomes Study Short Form-36) as the main instrument to measure health related quality of life. Significant influence of anxiety and depression on the quality of life in patients with both frequent episodic and chronic tension type headache was reported. Anxiety in these patients was mostly associated with psychic health, general health and vitality. In patients with chronic tension type headaches depression was associated with physical functioning; in those with frequent episodic tension type headaches — with social functioning and role functioning due to emotional sate.

Abstract

Genotypes and haplotypes frequencies for VDR polymorphisms in case-control studies of multiple sclerosis (MS) were investigated. Results show an association of VDR T/t variant with diseases (p < 0,05). This marker was also associated with amount of eosinophils in MS patients (p < 0,05). Polymorphisms B/b and F/f was associated with ESR level, IgG level and amount of the CD16+ cell (p < 0,05). We demonstrate that the Bft haplotype significantly higher observed at MS patients and btT — at healthy person Received data allow suggests that the allele t VDR and Bft haplotype of VDR gene could increase susceptibility to MS and have influence on clinical manifestation of diseases.

Abstract

The aim of the research is to investigate activation of antitumor immune response efficiency using professional antigen-presenting autologous dendrite cells in the complex treatment of patients with cerebral hemisphere malignant gliomas. We have analysed 48 cases of patients with malignant gliomas (35 — glioblastoms, 13 — anaplastic astrocytoma) who received an complex treatment with specific immunotherapy use. All the patients were hospitalized at late stage of disease and tumor size exceeded 5 sm. After the operation all the patients achieved satisfactory quality of life (not less than 80 points according to Karnofsky). Specific antitumor immunotherapy was conducted in accordance with original methodic, including autologous dendrite cells with tumor antigens and tumor cells lysate. Treatment course included intradermal introduction of dendrite cells at the 10—12 day after the operation. The treatment included not less than 3 injections. The course of treatment was repeated every 2—3 months. Specific antitumor immunotherapy included into complex treatment of malignant gliomas allows to increase disease remission, life quality being satisfactory.

Abstract

Taking medical documents as a basis we’ve set up the guantity of registered of primery brain’s tumour (BT) for the period of 1966—2005 years. The data were compared with the published accounts about the average guantity of solar spots, which reflects the level of solar activity. Mathematic analysis registered some parallels of dynamic summery freguency of all varieties of brain’s tumour (BT) and the quantity of solar spots , so that the changing of tumour’s graphic 4—5 years behind. The lagging of the curve of solar activity - sometimes as far a half of the 11 years cycle — refers only to benign tumour. Changing of the level of malignant tumour closely follows the solar activity index and have line interaction which depends on solar spots.

Abstract

The purpose of present research is to estimate the influence of ischemic stroke on changes of clinical and instrumental parameters in patients with chronic heart failure (CHF) depending on expressiveness of neurologic deficiency. We supervised 103 patients with CHF, among them 71 patients with stroke. In all the patients twice heart rate variability and echocardiography were carried out. To determine CHF FC we used an estimation scale of a clinical condition (SHOCKS by Mareev, 2000). After 6 months all the patients were divided into 2 groups depending on expressiveness of neurological deficieny. Significant deterioration of heart rate variability (HRV) parameters in the group of patients with expressed neurological deficiency was revealed. Also in this group increase of end diastolic volume, end systolic volume and deterioration of diastolic function was revealed. 6 months after stroke in patients with CHF with expressed neurological deficiency reduction of parameters of HRV, progressiveness of hyperactivity of sympathetic adrenal system, deterioration of morphological-functional parameters of left ventrical was revealed.

Abstract

The purpose of the study was to compare cognitive functions in patients with episodic tension-type headache (EpTTH) and chronic tension-type headache (ChTTH).

EpTTH was diagnosed in 34 patients in which it combined with psychovegetative syndrome in 17 persons and with cerebrovascular disorders in 28 persons. ChTTH was diagnosed in 57patients; it was associated with psychovegetative syndrome in 17 of them and with cerebrovascular disorders in 40 of them.

Difference between EpTTH and ChTTH patients was revealed only for the patients with psychovegetative syndrome. Such difference was slight and refered the tests for attention switching.. Patients with cerebrovascular disorders demonstrated worse modally non-specific problems with memory and attention and higher anxiety and depression levels that patients with psychovegetative syndrome.

Results suggest the different mechanisms of tension headache prolongation in the patients with psychovegetative syndrome and cerebrovascular disorders. The chronic tension headache itself doesn’t worse the cognitive functions while general causes exist which influence neuropsychological decline as well as tension headache stability.

Abstract

Main effects of system enzyme therapy (SET) are antiinflammatory, fibrinolythic and immunomodulating. Combination of these effects proved to be important in the treatment of 30 patients with stroke and 30 patients with ischemic cerebrovascular diseas. The use of polyfermental preparation flogenzyme in complex therapy allowed to normalize the state of thrombocytic hemostasis and to improve the indexes of plasmatic hemostasis and rheology of blood, serum concentration of proinflamatory cytokines (TNF-α, IL-1β, IL-4) to a considerably greater degree than in control group (39 patients) and impaired velocity and reactivity parameters (33 control). Thus, SET medications are necessary and very important pathogenetically in the complex therapy of patients with acute and chronic cerebral ischemia.

283 patients with gliomas were included in this study. Age, sex, neurological status and Karnovsky performance were analyzed before and after surgery, also tumor location, type and volume of surgical resection, postoperative complications were considered. Volume of tumor resection did not depend on glioma localization, excluding deep located tumors, in which case stereotactic cryotomy was performed (p < 0,01). In cases of stereotactic cryotomy postoperative neurological deficit worsening was noted in 12,5%, in patients with open biopsy and partial resection — 10,9%, and in case of total or subtotal tumor resection in 7,0% (p > 0,05). Partial gliom resection often related with postoperative complications and neurological deficit worsening then open surgery total tumour resection. Stereotactic cryotomy does not lead to bigger postoperative complications frequency in comparisons with open surgery.

Abstract

Extracerebral tumors of posterior fossa often lead to dysfunction of the brain stem, however the degree of brain stem’s damage is determined most often on the basis clinical data only. Magnetic Resonance Spectroscopy (MRS) allows to study biochemical processes and determine chemical content of the cerebral tissues noninvasively. Changes in brain stem’s perifocal zone in patients with acoustic neuromas and subtentorial meningiomas have been studied. Basic metabolite’s level’s analysis of N-acetyl-asparatate (NAA), creatine, choline and lactate have been conducted. Changes in brain stem’s metabolism depend on duration of the disease and the conditions of the patient on admission have been revealed. However the change in metabolite’s level and their role in the brain in case of pathology and in normal conditions need further investigations.

Abstract

A comparative clinical and serological analysis of two groups compatible by sex, age and clinical forms of acute tick-born encephalitis was conducted among 68 patients with confirmed immune enzyme method. All patients received a single injection of high titrating Russian human tick-borne encephalitis immunoglobulin by the doze varying on the patient’s weight and particular decease. All patients received symptomatic therapy as well. There were 33 patients in the group II who have been taking anaferon during two weeks since the acute period. The patients of the groups I and II didn’t have any robust alteration of the feverish period duration and cerebrospinal fluid sanitation but it was discovered that the patients of the group II with focal form had reveal predominance of IgM titer on the second week of the disease development comparing with the same indicators among the patients of the group I with focal form. The findings show the possibility to recommend anaferon for complex treatment of the patients with tick-born encephalitis in acute period.

Abstract

The aim of this investigation is to study the effect of BTA on the cerebral blood flow in patients with chronic daily headache. The analysis of Doppler ultrasonography and transcranial Dopplergraphy findings has shown the following: after the treatment 34% of the patients had no extravasal effect and in 66% of the patients it became moderate and after the treatment normal venous outflow was found in 58% of the patients . The results of the pilot study demonstrate the effect of BT-A injection on the cerebral blood flow by means of optimizing both the arterial blood flow and the venous outflow from the cavity of the skull.

Abstract

Basal-cellular skin cancer takes the first place accounting for 75—95% of malignant skin tumors. Despite different methods of treatment recurrence rate is up to 39%. Recurrent forms may invade soft tissues, major vessels, nerves and bones of the skull base. Options of surgical treatment of wide-spread recurrent forms are considered in this article. Modified block-resection of parotid-masticatory region along with temporal pyramid invasion is presented. Short- and long-term follow-up results are also presented.

Abstract

Brain-steam acoustic evoked potentials (BAEP) study was carried out in 84 patients with chronic-pain syndrome due to trigeminal neuralgia (TN). Preoperative BAEP changes were detected in all patients with TN. There were no specific differences in BAEP among variants of the disease course preoperatively. Percutaneous radiofrequency termocoagulation of Gasserian ganglion could be explored in all etiological types of TN. The lesion in trigeminal ganglion raised functional activity in the brain-stem part of the antinociceptive system. The radiofrequency lesion blocked the afferent pulsation from a peripheral part. It both relieved the severe pain and restored the damaged sanogenetic mechanism of pain suppression. Unrestorable BAEPs postoperatively were a prognostic factor for the pain relapse in patients with TN.
