Abstract

In the article the authors revise a number of methods used in clinic for the description of hypercoagulation syndrome and consider that persistent trombinemia signs and signs of platelet hyperaggregation with the increased consumption of them are the most reliable markers of hypercoagulation and prethrombosis state. Methodology of oncothrombosis prophylaxis and therapy is discussed. Authors demonstrate that heparin therapy decreases the risk of development of spontaneous and postsurgery thrombosis as well as of DIC-syndrome in cancer patients.
Authors define the ways of further optimization of thrombotic complications prophylaxis and therapy using thrombine inhibitors and the study of their effects on oncogenesis.

Abstract

Abstract

Polyphenol containing hepatoprotectors — maksar (complex of isoflavones and stilbenes of Far East plant — Maaсkia amurensis Rupr.et Maxim.) and legalon — during combined introduction with prednisolone to rats with chronic hepatitis, caused by tetrachloromethane, weaken the immunosuppressive action of glucocorticoid – augment the blood lymphocyte quantity, prevent the thymus atrophy, increase the blasts and antibody-forming cells content in spleen and the functional activity of with C3b-receptors in peritoneal exudation.

Abstract

Investigation of pathogenetic correlation of abdominal adisposity and II-type pancreatic diabetes (PD) has been made with the aim to reveal the importance of carbohydrate metabolism disturbances at above said pathology combination in cardiac abnormalities. 30 white alley rats at the age of 8-12 months have been included into the experimental group. Control group has been formed of 30 animals.
Used methods of investigation: simulation of II-type PD in rats with streptozotocin and study of carbohydrate metabolism indices in entire organism as well as contractile function indices and indices of isolated and contracting heart metabolism.
As a result it has been revealed that the weight of rats with II-type PD and abdominal adisposity, the level of glucose and glycated haemoglobin in blood, of lactate and pyruvate had been increased surely. The normal level of blood serum C-peptide has confirmed the absence of mass death of β-cells. The hearts taken from diabetic animals have responded to the increase of contraction frequency with the decrease of advanced pressure, i.e. the negative inotropic effect has been observed. Therefore the peripheral insulin resistance plays the leading role in the development of metabolic and functional abnormality complex at II-type PD and abdominal adisposity. Accumulation of lactate, metabolic acidosis, decrease of glucose efficiency and dysfunction of cardiac hystiocyte calcium pump with the development of diastolic myocardium dysfunction contribute to the development of metabolic disturbances.

Abstract

Epiphysis is an organ of the control of biorhythms and adaptogenesis connected with photoperiodism. To evaluate photoradiomodificating effect and conarium plasticity using the methods of light and electron microscopy, there have been analyzed changes in morphological indices of pineal secretion activity in white rat males after 48-hour bright light exposure, general X-ray exposure and its combination. The received results have been handled by the method of variation statistics. It has been revealed that the decrease of light pinealocyte part, its protein-synthesizing system reduction, reparative activity decrease and the increase of degenerating pinealocyte part after combined exposure were expressed more significantly as compared to the light and X-ray exposures.

Abstract

Cerebral hemodynamics state and peculiarities of bioelectric cerebral activity in patients with infectious mite borreliosis (IMB) have been investigated. 24 patients out of acute stage have been examined; the control group has been consisted of 10 healthy people. Values of rheographic index, index of vessel peripheral resistance, time of pulse wave spread, venous drainage index and elastic resistance index have been evaluated by rheoencephalography method (REG). ElectroEncephaloGram (EEG) has been registered in 12 abstractions according to the international system 10—20. Amplitude, frequency and indices of α-, β-, δ- and θ-rhythms, asymmetry degree and presence of paroxysmal activity have been evaluated. Use of EEG at patients with IMB has marked the α-rhythm suppression and increase of slow activity of δ- and θ-ranges, electric activity asymmetry and presence of paroxysmal discharges. By REG method there have been revealed expressed functional disturbances of cerebral circulation. Together with the impediment of venous drainage, this leads to the regulatory imbalance and the decrease of hemodynamic supply to separate cortex parts that may be a reason of bioelectric cerebral activity disturbances in patients with IMB.

Abstract

The aim of the investigation – to study changes in hepatic tissue resulting from the influence of low temperatures (– 195,8 °C) in experiments and clinic. Hepatic tissue investigations in experiment have been carried out using electron and light microscopy. During the surgery instruments of original design have been used – cryoscalpels and cryodestructors.
Experiments have been carried out on 45 sexually mature dogs of both sexes. Animals have been separated into 3 groups. On the first group (consisting of 12 animals), in the acute experiment, there have been studied features of cryoscalpel design characteristics. In the second series of experiment (on 22 dogs) there have been made a comparative study of hepatic resection using cryoultrasonic scalpel and simple surgical scalpel by means of the study of hemorrhage quantity. In 11 cases there have been studied hepatic morphological changes affected by cryoeffect. By the effect of low temperature ice crystals are appeared in hepatic tissue intracellularly and outside the cell, they cause mechanical injury and rupture of hepatic tissue. In 45 days connective tissue elements are formed at the place of cryodestruction.
The use of cryoultrasonic scalpel in hepatic resection in the experiment decreases the hemorrhage quantity by 1,26 times as compared to the traditional methods. Positive experimental investigations allowed to use the low temperature with success in clinical practice at 253 patients with different localized hepatic diseases with 13 lethal outcomes (5,1%). Thus, the used cryosurgical technique during surgical operations allows to decrease significantly the hemorrhage, to eliminate parenchymatous bleeding and to prevent relapses of disease both at growths and at parasitic hepatic affection.

Abstract

The results of investigation of adaptive properties at 577 children of junior school age (7—9 years old) are presented in the paper. Rise of physical growth, physical health index and adaptation level up to the 9 years has been revealed. The period of the least adaptability at girls (8 years old) has been determined. The sexual differences of the determined factors at the age of 8 and 9 years have been marked.

Abstract

In order to increase the efficiency of surgery at fallopian tube pathology there has been proposed a method of medical rehabilitation with the use of «Erectron» apparatus (autonomous electrical stimulator of gastrointestinal tract — AES GIT). In 1998—2002 in Tomsk Research Institute of balneology and physiotherapy 100 women at the age of 18—39 (26,3 ± 4,1) have undergone a cure with «Erectron» AES GIT. Postsurgical period has been 2,9 ± 0,2 months. After the course of electrostimulation there has been registered an improvement of gynaecological state, blood circulation and functional activity of fallopian tubes. Efficiency of treatment immediately after the treatment has been 84%. Pregnancy beginnings after 18 months — in 39% cases (control — 15%). The proposed method is simple, safe and easy of use and access. It can be used in physiotherapy departments and at the absence of such — as a little physiotherapy — in postsurgical room, medical assistant’s post and rural hospital.

Abstract

A clinical prospective examination of 90 women with complete pregnancy and indications for labor induction because of unsatisfactory maturity of uterus cervix has been made. The aim was to create a comparative analysis of efficiency of intravaginal introduction of prostaglandin synthetic analogue E1 misoprostol («Sytotec») and intracervical introduction of prostaglandin E2 dinoprostone («Prepidil» gel) for uterus cervix preparation and labor induction at complete pregnancy. Misoprostol in a dose of 25 mkg has been introduced to pregnant women of the 1 group (n=44), every 4 hours not more than 3 times. In case of discharge of waters or labor activity the second introduction has not been done. Dinoprostone has been introduced intracervically in a single dose to pregnant women of the 2 group (n=46). The use of misoprostol has been accompanied by spontaneous beginning of labor activity by 2 times more often than the use of dinoprostone. The quantity of vaginal births within 12 and 24 hours of observation has been surely greater and the duration of time between the beginning of introduction and labor has been surely smaller in the group of women received misoprostol as compared to the one received dinoprostone. It has not been revealed any differences between examined groups by the frequency of uterus hyperstimulation symptom development, labor duration, frequency of abdominal and vaginal labor, as well as perinatal outcomes.

Abstract

The aim of investigation: to ground the use of a new combined mud medication «Tone plus» in pulmonologic practice.
The investigation has been made on 53 patients with chronic bronchitis (CB) and bronchial asthma (BA). Inhalations with «Tone plus» consisting of three components — saline carrier, herb and peloids extracts — have been the main medication. Efficiency of the therapy has been assessed grounding on the movement of local respiratory tract protection indices by the method of examination of nasal tunica mucosa smear and nasal secretion, on the airway conductance by spirograph, inflammation activity by blood clinico-biochemical analysis and on organism adaptation system state by L.A. Garkavy’s method.
In atmiatrics there has been registered local and system anti-inflammatory effect, airway conductance improvement and improvement of upper airway cover epithelium state, increase of functional activity of respiratory tract tunica micosa cells.
Inhalations of a new combined mud medication «Tone plus» are a promising method of non-pharmacotherapeutic atmiatrics of patients with CB and BA.

Abstract

Investigations made on 96 patients with chronic Lyme-borreliosis accompanied with predominant joint affection have allowed to ground scientifically an advisability of two-stages treatment, with administration of physical factor complex at the second stage. A treatment technology has been developed that allowed to improve significantly the state of 82,4% of patients with good tolerance.

Abstract

An investigation has been carried out with the aim to reveal the main causes of temporary disability, invalidism and mortality of Siberian Chemical Combinat (SCC) workers.
There has been made an analysis of medical statistics data of the Central health department No 81 for the period of 1951-2001 and of results of psychophysiologic examination of workers in SCC nuclear- and radiation-hazardous areas.
Investigations have revealed that a radical (up to isolated instances) decrease of professional pathology occurrence caused by radiation and harmful chemical substance influence had taken place within 50 years of SCC functioning. Today psychosomatic disorders are the main cause of temporary disability, invalidism and mortality of nuclear- and radiation-hazardous plant workers.
Psychosomatic disorders occurrence and economic loss connected with it allow to specify the psychophysiologic support laboratories work on revelation and correction of inadaptation states at atomic industry workers as priority.

Abstract

Dynamics of neonatal mortality in Tomsk within the period of 1999—2001 has been analyzed in the article. Analysis has been made according to the T. Kern’s method by G.S. Muchiyev’s and O.G. Frolova’s modification. This method permits to evaluate the dynamics of neonatal mortality from the viewpoint of its averting and gives opportunities to reveal the errors at all stages of medical care of women and new-born children. 170 cases of neonatal deaths have been analyzed. There has been marked the decrease of both early and common neonatal mortality due to avertable reasons. Unavertable death reasons tend to increase due to congenital malformations. Ways of the further decrease of neonatal mortality have been defined.

In the article it have been systematized morphologic features allowing to define what body surface had been subjected to the crossing with motor transport wheels and the direction of crossing. Answer to this question is of great practical importance. 60 cases of determined automobile injuries (crossing a body) have been analyzed.

Abstract

The aim of investigation was a determination of ultrasonic and radioisotopic feautures of joint affection under Lyme mite borreliosis (LMB). There have been examined 84 patients by ultrasonography method and 24 patients by scintigraphy method. These patients’ diagnosis of Lyme mite borreliosis has been verified with the results of serological examinations. Changes of joint structures by arthrosonography results have been consisted of: effusion of migrating nature, joint capsule thickening, local synovial proliferation, hyaline cartilage thinning and ligamentous apparatus hardening. By radionuclide examination the greater quantity of affected joints has been revealed as compared to patient’s complaints and arthsonographic status, it has been confirmed by the further case monitoring. As a first stage of radiologic diagnostics algorithm of patients with joint affection under LMB it has been recommended the use of radionuclide examination that allowed to execute a topic diagnostics of affected joints. As a second stage it has been optimum to execute arthrosonography in order to evaluate the nature of joint affection.

The main task of non-state dental clinics is an active promotion of its’ services in the market. This thesis determines the typical models of market behavior. However, the making of dental service market is accompanied with a number of problems and their solution is a most important task of non-state dental clinic managers.
The development of dental clinics in modern conditions requires significant financial investment — into the purchase of building or facilities, purchase of modern equipment and staff training as well as advertising. Attracting of investment or long-term bank credits becomes the barest necessity. Forming of clinic attractiveness for investment for clinic development should be realized on the base of competent business-plan elaboration of a clinic development that demonstrates evidently the specific advantages of the project, the degree of risk, payback and profitability.

The system of medical users training organization promotes the use of Internet resources as a source of medical and lawful information.
