EXPERIMENTAL AND CLINICAL INVESTIGATIONS
153 patients with Q-wave noncomplicated and complicated myocardial infraction have been examined. Blood serum was studied for acute-phase proteins (alfa-2-macroglobulin (MG), alfa-1-antitrypsin (ATr), and lactoferrin (LF)) in 97 patients ((57.9 ± 1.06) years old). Blood was sampled at the 1st, 7th, and 14th days after myocardial infraction. The decrease of MG in acute phase was observed in patients with cardiogenic shock. Increased concentrations of LF and unchanged MG level at the 1—7 days were observed at pulmonary edema.
The influence of the total electromagnetic field on the concentration of air ions in the ambient air has been studied. The numbers of positive and negative air ions have been measured at a distance of 10 cm from the head surface of children 1—4 days and 2—4 years old. It has been found that the number of negative ions decreased on the average by 88% near a newborn baby оand by 9% near a child 2—4 years old. The number of positive ions decreased on the average by 34% near a newborn baby and remained unchanged near a child 2—4 years old. A newborn baby affects more strongly the ionic composition of the ambient air, because osseous structures and soft tissues of skull form a weaker barrier for the brain electromagnetic field.
The level of uric acid in the blood serum of 103 patients with coronary heart disease was researched in clinical conditions. The interrelation with the components of the metabolic syndrome in patients on the background of individually selected pathogenetic therapy was studied. It was shown that the abdominal obesity has the highest correlation with the level of uric acid in a cluster of metabolic syndrome components.
Allelic variants of genes involved in spermatogenesis can contribute to the genetic predisposition to idiopathic male infertility. In the present study we investigated the association of polymorphism rs3088232 in the BRDT gene with the risk of this pathology on the sample of 105 infertile patients and 230 healthy controls. We revealed the association of allele G (OR = 1.80; CI 1.16—2.80; p = 0.008) and genotype GG (OR = 6.47; CI 1.23—34.15; p = 0.01) with idiopathic male infertility.
Using electron microscopy and morphometric analysis studied the structural changes of neurons and radial glia of the retina of white mongrel male rats when exposed to light (6000 lux) in the dynamics after 1, 7, 14 and 30 days. The proposed mathematical model has allowed a high degree of probability to assess the state of a pool of cells of the retina at specific times throughout the experiment. The most striking structures of the retina are neurosensory cells, and neurons of theinner retinal layers are less sensitive to these influences.
We consider the nature of the brain bioelectric activity changes in the dynamics of heart rate variability parameters biofeedback sessions in teenagers. There was revealed increase in predominance of the brain functional activity and increase in the stability of the subcortical brain structures. The biggest bioelectric reorganizations are revealed in the right brain hemisphere, often involving the frontal brain lobes. These changes were more pronounced in adolescents with successful completion of training.
The rate of oxygen uptake by heart mitochondria (Mch) at different heart pathologies (myocardial infarction, diabetes mellitus and their combination) was studied polarographically. The rate of oxygen uptake of damaged heart Mch and content of free fatty acids (FFA) were increased. Change of these parameters was increased with the development of pathologies that is most pronounced in the groups «diabetes» and «diabetes + myocardial infarction». In the group of «myocardial infarction + diabetes» pathological process was developed less intensively in comparison with individual pathologies. Addition of FFA suppressed the rate of oxygen uptake of Mch. Equally, inhibition of phospholipase A2 suppressed the respiration of Mch. The hypothesis that FFA act as modulators of the activity of phospholipase A2 was discussed.
The activity of kallikrein-kinin system of blood and the state of placental circulation at full-term physiological pregnancy have been studied.
In 30 pregnant women aged 18—27 at 38th—40th weeks of pregnancy, the activity of kallikrein-kinin system (KKS) of blood before and after the action double local hypoxia of arm and the state of placental hemodynamics were analyzed. Three types of reaction of kininogenase and inhibitory chains of KSS were revealed: adaptive, kininogenase, and inhibitory. Disturbed placental hemodynamics was observed in pregnant women with kininogenase and inhibitory types of KSS reaction.
Analysis of clinical and electroneuromyographic performance of two groups of patients with secondary polyneuropathy showed that diabetic neuropathy affects the peripheral nerves of the lower extremities, whereas in polyneuropathy of mixed etiology (paraneoplastic and toxic) in the pathological process involved both nerve fibers, the low and upper extremities.
Peculiarities of the peripheral circulation of the first athletes from different directions of the training process by rheovasography. Changes at the peripheral level of the cardiovascular system in athletes, coaching speed and endurance, designed to create conditions enabling environment for maximum extraction of oxygen from the blood into the tissues, which manifests itself in reducing zhenii-tone of blood vessels and reducing the vascular tone at the exchange level.
In the formation of metabolic syndrome there is a change of the quantitative characteristics of immunocompetent cells and blood cytokine profile. According to our study, the elevated serum content of inflammatory cytokines (IL-6, IFNγ and TGF-β), in the cases of IL-10 decrease, was detected in patients with metabolic syndrome. The level of serum IL-1 inpatients with metabolic syndrome was within normal parameters. With it, there was a significant decrease in the percentage of blood CD3- and CD4-T-lymphocytes, and, conversely, an increase in the number of activated T (CD25+) and B (CD23+)-lymphocytes and monocytes (CD14+) has been in patients with metabolic syndrome. This revealed changes, is characterized by the presence of subclinical chronic inflammation in metabolic syndrome patients. It can be the result of compensatory immune responses developing in the weakening of adaptive immunity.
The results of experimental researches showed that extracts from Delphinium elatum L. have moderate antiproliferative activity, essentially decrease progress of inflammation, induced by Freund’ Adjuvant and development of germs.
Biocompatibility of original implant calcium-phosphate materials was evaluated in the experiment on animals. The methods of radiological electron-probe microanalysis (REMA) and light and scan electron microscopy (SEM) were used. Studied materials had the properties of biodegradation, osteoinduction and osteoconduction at different extent. The materials with the composite maximally close to the the bone tissue had the greatest grade of biocompatibility.
The investigation was dedicated to the exploration of reamberin influence on oxidative stress and metabolic disturbances in 64 patients with abdominal sepsis. The authors used reamberin for correction of oxidative processes and metabolic disturbances for treatment of the patients with abdominal sepsis. The authors registered that reamberin improved the oxygen-transport function of the blood and metabolic processes during the abdominal sepsis via the hybernation condition — a form of adaptive reaction of organs and systems during the endotoxemia for creation of more effective environment for functioning of main organs and the whole organism. The authors show that the main pathogenetic mechanisms for multiorgan dysfunction are metabolic disturbances and energy deficiency in organs and systems with subsequent tissue hypoxia.
Some biochemical indicators of peripheral blood and pleural fluid (level of the total protein, glucose, lactate dehydrogenase activity) at patients with various variants of tubercular exudative pleurisy (MBT-positive and MBT-negative) for the purpose of definition of possibility of use this indicators as criteria for differential diagnostics of a pleurisy, have been studied. It has been established that tubercular exudative pleurisy is characterized by the expressed changes of the biochemical status of blood and pleural fluid in comparison with that at healthy donors and patients with not tubercular exudative pleurisy, however, this changes in most cases are nonspecific and don't depend on a variant of tubercular exudative pleurisy.
LITERATURE REVIEW
The most plausible formation mechanisms of the secondary immunologic insufficiency and dysregulation of immune response to M. tuberculosis in relation to the biologic specificities of the infectious agent are discussed in this review. The ability of M. tuberculosis to affect the activity of antigen-presenting cells and T-lymphocytes, which manifests itself in disbalance of pro- and anti-inflammatory cytokines and the expression of the basic molecules of intercellular cooperation, the activation of the suppressing mechanisms and apoptosis resulting in immune hypo- and anergy of T-lymphocytes and the disorder of effective immune response formation in general, is also described.

This publication shows the comparative capabilities of localized and advanced volume ultrasound in emergency conditions of the abdominal cavity. To achieve this goal, a retrospective analysis of ultrasound examination of 130 patients with OAB in LOAC — 46, NCP — 33, LVK — 14, LNC — 18. The sensitivity of the localized and extended survey of the composition, respectively, in the AHC -81% and 93%, OMV — 33% and 77%, LVK — 50% and 86%, LNC — 53% and 73%. Comparison of the sensitivity of ultrasound examination, depending on the localization of SAR and sonography showed an increased volume of ultrasound can improve the sensitivity of the examination in patients with acute pathology of abdominal organs from 12 to 44%.
The efficiency of treatment of patients with allergic rhinitis and associated bronchial asthma by physical factors and advisability of two-stage treatment are justified. Sixty six patients were observed. The conclusion on the efficiency of treatment was based on the data about the dynamics clinical state of the primary and associated diseases, intensity of inflammation activity in respiratory tract
(rhinocytograms, biochemical indices of nasal lavages, level of nitrogen oxide in expired air were studied), patency of airways (manometry, spirography). We compared two versions of treatment: two- and one-stage. Patients of the first group (30 patients) received elimination therapy for 3 to 5 days with the following peaty mud application, current of supersonic frequency, massage, inhalations, physical therapy. Patients of the second group (36 patients) did not receive elimination therapy. We have found a decrease in intensity of clinical implications of the primary and associated diseases, decrease in inflammation activity, better nasal and bronchial носовой patency, improvement of the mucociliary function in patients of the first group. In the second group, the recovery of the surface epithelium and nasal patency was not observed against the background of improvement of the clinical state.
Presents the results of a comparative study of combined therapy (immunoglobulin against tick-borne encephalitis in conjunction with reaferon-ES-lipint) and base treatments (immunoglobulin against tick-borne encephalitis), meningeal form of tick-borne encephalitis. It was shown that complex therapy contributed to greater clinical efficiency and improve the indicators of cellular immunity, was safe and had no side effects.

Comparative epidemiological research on studying prevalence tobacco smoking and respiratory symptoms among rural teenagers with bronchial asthma. Control group have made non-smoking (n = 59), and skilled — smoking teenagers (n = 72). Prevalence of smoking among adolescents with bronchial asthma has made 55% against 44,3% on the average at adolescents (p < 0.05). At smokers sick of a bronchial asthma the presence of respiratory complaints, level of СO in exhaled air and urinary cotinine has been raised. Authentic communication of smoking with a series of symptoms bronchial asthma and chronic bronchitis in teenagers, shown is taped by a high and average parity of chances.
Score of the role of the contractile reserve of the left ventricular free wall in patients with ischemic and dilated cardiomyopathy was investigated for the selection of candidates for cardiac resynchronization therapy (CRT). Local reserve contractility was studied by assessing the dynamics of myocardial systolic velocity by tissue Doppler pulsed-wave at rest and at peak postural tests. Preservation of contractile reserve is the most important factor for response to СRT compared with dyssynchrony.
Potentialities of 199Tl-chloride and 99mTc-MIBI scintigraphy in diagnostics of musculoskeletal system inflammatory and tumor processes were assessed. Nonspecific indications of inflammations and tumors were obtained in 92.6 и 98.5% respectively. Accuracy 199Tl-chloride and 99mTc-MIBI scintigraphy in diagnostics inflammations were 83.7 and 73.2% respectively, tumors were 88.8 и 75.6% respectively. These markers both are available for indications, and 199Tl-chloride for differentiations of them.
ECONOMICS AND HEALTH CARE MANAGMENT
The Tomsk Region is a zone of average risk multiple sclerosis (MS), prevalence makes 27.1 cases on 100 000 people. During the period with 1980 for 2010 variety of morbidity from 0.4 (1992) to 4.1 (2003) on 100 000 people. The lowest morbidity was in period with 1991 for 1995 and makes 1.8 on 100 000 people, and the highest in the rage from 2001 for 2005 — 3.1 on 100 000 people. Last 5 years in the TomskRegion to increase ofmorbidityis observed.
The results of prevalence, structure and timerisk study in children birth with malformations inOmskin the period of 1998—2008 are presented. Birth of children with congenital abnormality weight average index accounted for (47.72 ± 0.66)%. Musculoskeletal and cardiovascular malformations prevailed in birth abnormality total number structure. According to ICD-10 the congenital malformations were united in groups and seasonal variation indices nave been determined for them. Study results indicated the predominance of children conception cases with high risk malformations forming in August, November and December.
Comparative retrospective study of emergency care rendering results for 1200 patients with uncomplicated fibrillation attacks was carried out and their management tactics clinic-economic assessment at a pre-admission stage was done. It has been found that therapy for atrial fibrillation is unproductive in the first hour of observation at a pre-admission stage. Atrial fibrillation attack was eliminated to the end of the first day regardless of used antiarrhythmic preparation in more than 90% of the cases. Proposed management tactics for patients with uncomplicated atrial fibrillation attacks until 24 hours of duration at a pre-admission stage allows by cost lowering and clinical effect of conducting antiarrhythmic therapy maintenance to shorten an emergency team stay time and increase its operative turnover.
The article includes information about the prevalence of asthma, allergic rhinitis and eczema among childhood in the program of the International Study of asthma and Allergy in Childhood — ISAAC. The present results of skin prick-test to describe sensitization in boys and girls 7—8 and 13—14 year old, living in different environments. The results indicate a lower prevalence of skin allergy in rural areas.
Within the limits of the regional program «Prevention and treatment of an arterial hypertension for the period of 2004—2008» the electronic register of the patients with hypertensia inTomskRegion has been created.
The electronic register is a two-level system where interaction of two kinds of databases is carried out: the first level is the databases of separate medical organization; the second level is the central integrated database.
The basic information for the electronic register are documents confirmed by the Health service Ministry of the Russian Federation, that is the coupon of the out-patient patient and a card of dynamic supervision over the patient with hypertensia.
All the data about the patients, included in the register are subdivided into unchangeable and changeable ones.
The electronic register is an effective control system providing local leading of health service bodies with qualitative and high-grade information in processes of preparation of decision-making and measure taken for prevention and treatment of hypertensia.

The period of electric activity of muscles is shortened in the group of high-qualification dancers. This is obviously connected with the shorter time needed to perform a motion, because the synchronization of motor fibers (by the mean frequency) increases and, as a result, the period of electric activity decreases. Consequently, just the character of bioelectric indices of rectus muscles of thigh determines the level of special motor fitness in sport dances: the higher is the dancer skill level, the more pronounced in the difference in the amplitudes and mean frequency of second realization.
Antioxidant complex under the experimental conditions in male rats of Wistar effectively reduces the severity of adhesions in the abdominal cavity. The morphological study showed that the mechanism of action of antioxidant complex is aimed at reducing inflammation, edema and improvement of microcirculation in the zone of damage.
The aim of this study was the estimation of the influence of liposomal ribonucleic acid (RNA) medicine «Liprina» on interferon status of women with recurrent genital herpes. In this study 60 women were included, who combined (acyclovir and Liprina, n = 40) or monoterapy with acyclovir (n = 20) were received. The levels of serum interferon alpha and gamma along with cervical virus elimination were estimated. The medicine «Liprina» increased the therapy efficiency of the women with genital herpes, that perhaps related with endogen interferon production amplification.
The content of essential (Zn, Se, Cu) and toxic (Pb, Cd) microelements in blood serum and daily urine of 120 children from
3 to 15years old with microbal-inflammatory nephropathies was determined. The deficit of essential (Zn, Se, Cu) and excess of toxic (Pb) microelements in children with acute and chronic pyelonephritis at different periods of the disease were revealed.
The results of the dispensary control for 188 patients with drug-resistant tuberculosis having been analysed during 3—5 years; those patients hadn’t received the adequate chemical therapy: 50% — died because of growing more serious tuberculosis in the first 2 years of the control, 33% — the process became cronic, 7,4% — self-recovered, 9,6% — left for other places or for the prison hospitals. Some patients are known to infect with drug-resistant strains MBT nearly 1 500 people every year, thus keeping the circulation among town population DR/MBT as well as the high level of drug-resistant tuberculosis cases.
DISCUSSIONS
This review presents new data in respect of polyfunctioning of the muscular system. It is shown that the muscles are able to synthesize and секретировать миокины, with auto-couple-and endocrine effects. Is represented by the signal cascade, which runs the transcription of interleukin -6 (Il-6) in the skeletal muscles. For example, IL-6, as an important миокина, shown its metabolic and антивоспалительные effects at physical loads. Considered the issue of the specificity of the cytokine response in muscle contraction. This position is confirmed by the fact that, in эндотоксемии induced by the introduction of липополисахарида, the muscles begin to produce, as the IL-6, and factor of tumor necrosis α. The biological sense of the rise of production of IL-6 and other миокинов is that миокины are the communication system, the input signal of different organs and systems, primarily in the liver, adipose tissue, and immune and endocrine, subordinating their work requirements of long-term muscles flex.
