Abstract

It has been revealed that blood system changes observed under the impact of pathogenic factors, different by their nature, and mechanisms laying in their basis were, in many respects, of the same kind. However the specific blood system response is defined by the nature of the acting exciter owing to specific changes in regulation structure functioning. Therefore changes in white blood, and at the acute hemorrhage – in red blood, prevail at inflammation during stress along with the proportional stimulation of different medullary hematosis outgrowths. Repeated extreme impacts lead to the emaciation of adaptive abilities of hemopoietic tissue as a result of the expressed hemopoiesis rocess irregulation.

EXPERIMENTAL AND CLINICAL INVESTIGATIONS

Abstract

The aim of investigation is to define the peculiarities of immune system functioning of patients with endometrium cancer depending on the tumor differentiation grade. 133 female patients of comparable age in menopause have been examined: healthy women, patients with histological verified high-, medium- (1-st group) and low-differentiated (2-nd group) endometrium adenocarcinoma of the I stage. Estimation of the immune state has been made in accordance with generally accepted recommendations. Immune system activation has been stated in the 1-st group, in the 2-nd group there has been marked a dissociation of the function of basic immune system chains and a decrease of their functional abilities.

Abstract

Concentrations of cytokines (IL-1β, IL-6, TNF-α) of α2-macroglobulin (MG), α1-antitrypsin (AT) plasminogen (PL), whole protein, albumin and uric acid in blood serum of patients with collagenosis have been investigated aiming the study of their complex interaction and the possibility of their use during differential diagnostics. The blood serum of 60 healthy donors, 53 patients with rheumatoid arthritis (RA), 15 patients with reactive arthritis (REA) and 16 patients with systemic lupus erythematosus (SLE) has been studied. IL-1β, IL-6 and TNF-α concentrations have been defined by ELISA, MG, AT and PL-rocket immunoelectrophoresis, the whole protein, albumin and uric acid — by biochemical methods. The albumin level decreased in all groups of patients. The whole protein concentration decreased at the first RA activity degree. MG, AT and PL levels had no difference at all diseases as compared to the control group. IL-6 concentration increased significantly at all patients groups. TNF-α increased with the RA severity but differed statistically significantly from REA and SLE only at the most severe degree. Analogous trends in IL-1β concentration have been found in cases of RA and SLE but at REA great individual fluctuations with the high average level have been found. Synchronous change of the studied cytokine concentrations without associated MG level change is evidence of the damage of traffic and regulatory functions of this protein. The uric acid can be used for SLE diagnostics and the dynamic supervision of IL-1β and TNF-α can be a prognostic criterion at RA.

Abstract

Morphofunctional state of left ventricle (LV) myocardium and right atrial (RA) auricle at 31 ischemic cardiomyopathy patients with chronic cardiac decompensation has been investigated in details. Histological preparations colored with hematoxylin and eosin and by Mallory method have been studied with usual light and polarization microscopy. The correlation of morphofunctional state of ventricle myocardium and atrium has been found, the factors of unfavorable farther results of operational treatment of this patient category have been revealed: presence of morphological myocarditis picture and marked LV myocardium fibrosis were pathomorphological predictors of LV dilatation in late postoperative period.

Abstract

A fundamentally new method of the treatment of patients with a profound degree of deafness – cochlear implantation – has been described. During operation a system of electrodes providing the sound perception by electrical stimulation of survived auditory nerve fibers is to introduce into the patient’s inner ear. The scheme of cochlea implant has been presented, the selection of patients and postoperative rehabilitation stages have been described.

Abstract

Analysis of pregnancy course, labor and early neonatal period at 294 women and their new-born children has been made for the estimation of the efficacy of different antibacterial therapy variants of urogenital clamidiosis during the second half of pregnancy period. The danger of abortion and chronic intrauterine fetal hypoxia are most often gestational process complications revealed against chronic urogenital clamidiosis. The use of sumamed is course dose of 3 g is an optimum pharmacotherapeutic regimen during the second half of pregnancy period.

Abstract

The aim of investigation is to compare the rate of ageing at patients with stomach ulcer (SU) and of practically healthy people. Dynamics of biological age of 406 patients with SU aged 40—59 years and of comparable practically healthy groups has been observed. The ageing of patients with peptic mediagastric pyloroduodenal ulcers develops faster than of practically healthy people. The rate of ageing at patients with peptic mediagastric pyloroduodenal ulcers depends on the clinical course and effected therapy.

Abstract

In the paper a popularity frequency and peculiarities of depressive-troubled disorder (DAD) course at patients with bronchial asthma (BA) have been estimated depending on the presence of associated erosive-ulcerous affections of gastroduodenal zone (EUA GDZ) and an impact of DAD on BA course has been studied. Depressive-anxious disorders have been revealed at 87 (78%) of 112 interrogated patients, most of them had the depression of medium degree that had required a therapy with the possible antidepressant use. Combined disorders in the form of depression and anxiety prevailed. It has been found that persons of the older age were subjected to the risk of DAD development. Patients with mixed pathology had more expressed DAD.

ECONOMICS AND HEALTH CARE MANAGMENT

Abstract

Field medical aid is most effective in providing the population of northern areas of Tomsk region with qualitative and accessible medical services. Development of such organization form is the most perspective way of health protection system perfection in the region. The most results will be achieved while providing the interaction and coordination of the activity of territorial treatment-and-diagnostic institutions and field medical teams. The field forms of aid should complete the activity of regional hospitals, provide the accessibility of medical services in remote areas and equalize the quality of medical aid in areas particularly regarding high-tech treatment-and-diagnostic procedures.

Abstract

A clinical, electroneuromyographic (ENMG) and pathomorphological investigation of 19 eliminators of accident consequences (EAC) in Chernobyl APP in 1986 and 27 Tomsk region inhabitants living in the accident area that has taken place at radiochemical plant of Siberian Chemical Complex in 1993 has been made with the aim of the influence study of low ionizing radiation levels on the peripheral nervous system. Symptoms of sensory polyneuropathy prevailed in both groups clinically. Mixed affection type has been found at EAC by ENGM method, affection of myelinic nerve fibre membrane has been found at people living in accident trace area. Morphofunctional changes in skin allow considering the role of immune system in mechanisms of neuroglial and neuronal damages, distant by time.
