Фибрилляция предсердий и ишемическая болезнь сердца: фокус на эпидемиологию и патофизиологию в рамках коморбидности

Аннотация
Фибрилляция предсердий и ишемическая болезнь сердца (ИБС) часто встречающаяся и клинически значимая коморбидность. С учетом различных эпидемиологических исследований есть мнение, что взаимосвязь между фибрилляцией предсердий и ИБС сложна и гораздо теснее, чем простое совпадение. Эти два заболевания имеют общие факторы риска, влияющие на соответствующие механизмы развития данных состояний, что способствуют формированию порочного круга их возникновения и прогрессирования. В таком случае ИБС может быть как причиной, так и следствием фибрилляции предсердий.
Целью данного систематического анализа являются оценка и сравнение мнений авторов различных публикаций о взаимном влиянии фибрилляции предсердий и коронарной болезни сердца с учетом эпидемиологии и их патофизиологических взаимодействий.
Материалы и методы. Для отбора необходимых публикаций была использована методология PRISMA 2020 (Preferred Reporting Items for Systematic Reviews and Meta-Analyses). Согласно данному руководству, в поисковых системах PubMed и eLIBRARY.RU обнаружено 597 статей по заявленной проблеме. На финальной стадии в обзор включено 74 англоязычные публикации и 16 на русском языке. Существующие мнения в различных публикациях часто дополняют друг друга, но иногда носят противоречивый характер, что требует дальнейшего углубленного исследования в этом направлении. Результаты патофизиологических находок позволяют сделать вывод, что эти два заболевания способны образовывать порочный круг, усугубляя друг друга. Понимание взаимосвязанных механизмов позволяет перейти от симптоматического лечения к патогенетически ориентированной стратегии, направленной на устранение порочных процессов, связывающих фибрилляцию предсердий и ИБС.
