EXPERIMENTAL AND CLINICAL INVESTIGATIONS
Designing a hybrid vascular graft using biocompatible and biodegradable polymers is one of the ways to fill the market with vascular grafts of a small diameter necessary for coronary artery bypass surgery. Good adhesive properties of cultivated cells and low cytotoxicity of biopolymers can improve biocompatibility of polymer-based vascular grafts. The impact of protein modification of membranous polyhydroxyalkanoate and polycaprolactone scaffolds surface on their adhesive properties and cytotoxicity to multipotent mesenchimal stromal cells was evaluated. We used the following compositions of membranous scaffolds: № 1 polyhydroxybutyrate valerate ММ 2 307 kDa with polycaprolactone ММ 80 000 kDa, № 2 had the same composition but was made using a magnetic stirrer, №3 polyhydroxybutyrate ММ 541 kDa with polycaprolactone ММ 80 000 kDa and № 4 polycaprolactone ММ 80 000 kDa only. Premodification of scaffolds surfaces with 75, 50 and 25% fetal calf serum was found to significantly reduce cytotoxicity of scaffold № 4 and improve the adhesive properties of scaffolds № 1, 2 and 3. Premodification with fibronectin (10 µg/ml) mostly improved the adhesive properties of scaffolds № 1—4.
Study the dynamics of oxidative stress in the blood of patients with ischemic heart disease before and after on-pump coronary bypass surgery. 27 ischemic heart disease patients were examined. Serum lipid peroxidation product concentrations, total antioxidant activity and α-tocopherol levels were measured. It has been found that lipid peroxidation product concentrations in the blood of ischemic heart disease patients increased significantly along with the decreased activity of antioxidant system. After coronary artery bypass surgery lipid peroxidation — antioxidant system imbalance worsens. The severity of the found impairments is closely related to the functional class of ischemic heart disease.
The purpose of the study — to develop the program of the treatment on pulmonary tuberculosis (TB) sufferers with bronchoobstructive syndrome (BOS). 310 patients are examined under the offered diagnostic program including questionnaire, complex of the functional methods, study prooxidant status. It is developed basic medical program, in which different methods of the elimination of the reasons BOS are combined: adequate anti-TB chemotherapy, influence by pathogenic drug, modern schemes bronchodilator therapy, struggle against smoking.
On the basis the analysis of morphometric data are revealed the special features of the functional state of pineal gland in mice with the delay of sexual ripening. Is made conclusion about the involvement of pinealocytes into the regulation of reproduction.
Parametrs of action of electromagnetic radiation UV (320 nanometers) and IR (900 nanometers) spectrums, on cells of biotissue (cells of spleen, tumor and a cells bone marrow mice C57BL/6) are investigated. The research data show that action by infrared and ultraviolet radiances allow to regulate viability of cell population significant. In one case to raise their number (at an irradiating the infrared spectrum), in other to reduced their viability cells (at an irradiating electromagnetic waves of an ultraviolet range). The multiple increase of the present effects is displayed at an immobilisation of cells on an porous powder and porous-permeable incubators from a titanium nickelid.
The object of this work was to study the correlation between the lymphocyte proliferation and apoptosis in the cultures stimulated by specific inactivated Borrelia garinii antigen and the different clinical outcomes in Lyme borreliosis patients. It was found that borrelia antigen stimulation of the cultures obtained from chronic patients induced T helper type 1-like response as well as depression in lymphocyte apoptosis, which may provide the basis of persistent activated T cell immune response and chronic inflammatory in Lyme borreliosis.
The influence of capnographic training with biological feedback (BF) on character of leg hemodynamics and physical performance has been studied in sportsmen training in cyclic sports. The capnographic FB training favors an increase in the efficiency of oxygen use by muscles, optimizes the level of tissue respiration, favors the faster adaptation of blood flow to metabolic needs of leg skeletal muscles, and, finally, provides an increase in the physical performance of sportsmen. The results obtained allow us to consider the capnographic training with biological feedback as a promising component of the training process in cyclic sports.
The aim: to study the influence of P. aeruginosa exometabolites on E. coli lux+ bioluminescence and experimentally prove the expediency of use of a bioluminescent method for an estimation of their potential pathogenicity. Objectives and Methods. Exometabolites of P. aeruginosa (n = 27) nosocomial strains were used. Index of potential pathogenicity (IPP) was calculated with the formula: IPP = (Ik — Io)/Ik × 100%, where Ik — intensity of Escherichia coli lux+ bioluminescence in control test (LB), Io — intensity of a bioluminescence in experimental test (exometabolites). Results. Protocol of analysis implementation and exposure duration has been determined. According to IPP, P. aeruginosa strains were subdivided into 3 groups: strains with low degree of pathogenicity, moderate and highly pathogenic. The group of IPP ³ 70% was characterized by reliably high correlation coefficient relatively to pyocyanin production, film-forming ability. This indicates the possibility of bioluminescent method application as screening or additional method of the assessment of P. aeruginosa isolate aggressiveness, and in purulent-septic infections (PSI) in particular.
The processes of regeneration of the damaged rat bottom jaw bone after application of enriched platelets a fibrin clot were studied by morphological and radiovisiographic methods. After operation with use of fibrin technologies already after 1 week the bone tissue defect is filled by the merged islets of again generated bone. By second week after fibrin use the further formation of bone tissue in defect and formation of a bone callosity is noted.
With attracting contemporary molecular-genetic and immunological research methods, the article analyses etiopathogenetical factors of formation of eosinophilic blood reaction under pulmonary tuberculosis. It has been established that blood eosinophilia under pulmonary tuberculosis is reactive, mediated by hypersecretion, blood cells of IL-5-key eosinophil-activating mediator as well as excessive expression of IL-5R on the membrane of eosinophilic granulocytes. Besides, susceptibility to the emergence of eosinophilic blood reaction under tubercular infection is associated with the homozygous genotype CC and allele C of the polymorphic region C-703 T of the gene IL5 as well as with allele A and genotype AA of the polymorphic region G-80 A of the gene IL5RA.
An important role in anti-infection protection during pregnancy belongs to local immunity mechanisms. At contamination of lower genital tracts by Ureaplasma urealyticum and Mycoplasma hominis, the activation of natural killers in basal decidual membrane was absent, which can be considered as a marker of immune compromise of women in the aspect of reliability of control of the population of urinogenital opportunistic flora and prognostic factor of possible intrauterine infection.
The investigation results showed carbon monoxide had antiproliferate potential in Jurkat cells. This gaseous transmitter led to delay of G1 phase of cell cycle. Inhibition of p38 MAPK-dependent pathways didn’t influence on CO-mediated proliferation of Jurkat cells changes.
The clinical trial of 60 patients (mean age (28.5 ± 0.9) years) with moderate severity hepatitis A (VHA) has been performed. The diagnosis in all patients was verified by detection of IgM to VHA by ELISA. In the course of the disease clinical symptoms and functional status of the liver were evaluated. We determined the activity of alanine transaminase, aspartate transaminase, gammaglutamyl transferase, alkaline phosphatase, and the content of bilirubin. Malondialdehyde (MDA) level in blood serum was determined as a laboratory indicator of endogenous intoxication. Main group consisted of 30 patients treated with antigomotoxic therapy, whereas in the control group (30 patients) the common therapy was carried out. A faster elimination of clinical and biochemical manifestations of the acute phase of VHA was established in the main group compared with the patients of the control group. In contrast to the latter, the average MDA in the main group patients 3 and 6 months after treatment did not differ significantly from the normal values.
Jurkat tat cell line reaction on short-term (24 h) in vitro contact with model composite implants designed for bone tissue bioengineering has been studied. The titanium samples was coated by thin nanostructured calcium phosphate (CaP) layer was formed by rf-magnetron sputtered deposition. An increase in the number of leukemic line T-lymphoblasts presented CD124, CD25, CD4, CD8, CD16/56 markers of cell maturation and differentiation has been revealed with the help of Wilcoxon T-test. In a case of CD8 positive cells the differences reached to statistic levels (Mann—Whitney U-test) as compared with cells had no contact with model implants. No implants effect was mediated by cytokines secretion (IL-1b, IL-2, IL-4, IL-6, IL-8, IL-10 and TNFα). An elevation of apoptotic and necrotic cells number has been not revealed. Silicone incorporation into CaP composition had no essential modulating action on indices studied. A conclusion about an absence of immunotoxic effect of investigated coatings as regards to T-cell immunity has been made. In vitro acceleration of CD8 presentation by human T-lymphoblasts may be conditioned by their direct interaction with artificial CaP surfaces.

В статье рассмотрены условия, необходимые для создания теории самостоятельной механической активности легких. Кроме личности исследователя важнейшим условием была развитая научная школа. Гипотеза возникла в результате наблюдения парадоксальных фактов при исследовании механики дыхания и кровообращения в малом круге. Систематизация парадоксальных фактов, изучение основных парадоксов, имеющих фундаментальное противоречие парадигме Ф. Дондерса, в клинике и эксперименте на животных позволили получить доказательство самостоятельной активности легких и сформулировать теорию. Доказательство механической активности легких основано на учете фундаментального первого закона термодинамики, согласно которому в структурах легких функционирует источник механической энергии, вызывающий парадоксы и функционирующий только при жизни. Поиск источника механической энергии и изучение регуляции его функции составляет новое научное направление, требующее создания новых технологий исследования.
Природа механических движений внутренних органов остается не изученной с позиции классической механики. В частности, не известен источник механической энергии, расширяющий бронхи, сосуды, органы системы пищеварения и обеспечивающий механическую диастолу сердца. Изучение механики движений внутренних органов с позиции классической механики аналогично результатам исследования механики дыхания позволит открыть пока не известные, гипотетические фундаментальные свойства органов и новые направления изучения физиологии механических движений внутренних органов.
LITERATURE REVIEW
Genetically determinate and acquired disorders of immune reactivity in tuberculosis infection are discussed in this review. The changes in immunocompetent cells, manifested in a disorder of the structure of their cell membranes, proliferation processes, intracellular metabolism and functional activity, which cause reduced resistance of the organism to infection with Mycobacterium tuberculosis and contribute to the formation of secondary immunological deficiency in pulmonary tuberculosis are described.

In experiment on 20 mongrel dogs demonstrated the possibility of forming a comprssion enteric anastomosis in acute mesenteric ischemia using nikelid-titanium implants. In order to control vascularisation anastomosises sites it is applied transilluminations angioscopy. Determined by mechanical strength and terms of an inconsistency anastomosises, terms of tearing away of a design are defined. Carrying out possibility keeping bodies operations on a small intestine and character of healing anastomosis as a primary tension is proved.
A retrospective analysis of clinical and laboratory data of women with inflammatory diseases of the pelvic organs was performed. It was shown that the inflammatory changes of genital tract and pelvic adhesions lead to the development of autoimmune ovarian lesions.
One of leading aspects of modern surgery is development органосохраняющих technologies of surgical interventions. Besides very actual the economic aspect of new operations is represented. Modern криотехника allows to introduce in a clinical practice new methods of surgical treatment of diseases of the pancreas meeting practically all requirements of modern surgery and appropriating existing standards of treatment of diseases of a pancreas. The important advantage of offered operations is also decrease in probability of progress of heavy and dangerous postoperative complications, reduction of residence time of the patient in a hospital after operation, restoration of work capacity of the patient and a possibility of restoration of the active social status.
FROM SCIENCE TO PRACTICE
Absorption spectra of breath air for 108 healthy persons, 28 patients with pulmonary tuberculosis and 56 patients with many a diseases were recorded using laser photoacoustic technique. It was ascertained that, as the breath air analysis result, pulmonary tuberculosis can be diagnosed.
The purpose of the studies was to determine pathogenetic significance of some immunologic and biochemical indicators of blood serum in patients with skin lymphomas (SL) based on the anamnesis findings and the presence of concomitant internal pathology. General clinical, immunologic, biochemical and histologic studies were carried out in 89 patients. Out of them, T-cell lymphomas — 32, B-cell lymphomas — 18, chronic dermatoses — 32 were diagnosed. It has been shown that an integrated risk factor (IRF) for LS occurrence is the highest with the pathology of barrier lymphoepithelial organs (IRF 12,66), the development of elevated sensitivity to divers types of allergens and antigens (IRF 9,03), long-term intoxication by occupational hazards (IRF-4,5). Prognostic indicators (PI) for SL development rise with disorders in metabolic (PI 14,99), detoxication (PI 14,13) liver functions, with virus infections (PI 9,71), drug misadministration (PI 6,32), hyperinsolation (PI 4,17).
The aim of the study was to evaluate the efficacy of lecithin/cholesterol liposomes in the therapy of experimental venous congestion-induced chronic liver insufficiency. The effects of the empty liposomes, ATP and phosphocreatine-containing liposomes and alpha-tocopherol and superoxide dismutase-containing liposomes were studied. It was found that liposomal drugs have a hepatoprotective effect, i.e. they decrease blood enzyme activity (cytolysis markers), reduce oxidative stress and preserve histologic integrity of the liver.
Variations of cerebral blood flow have been studied during a day with the use of rheoencephalography and Voll diagnostics in healthy teenagers and teenagers with consequences of natal trauma of cervical spine. The studies have proved that teenagers with trauma consequences had amplitude and phase desynchronosis in all REG parameters and electrical conductivity of some points of Voll meridians. The determination of the time, at which extreme REG values (acrophase and bathyphase) take place, with the aid of cosinor-analysis forms the basis for more correct scheduling of procedures during a day for teenagers with consequences of natal trauma of cervical spine.
ECONOMICS AND HEALTH CARE MANAGMENT
The comparative clinical analyses of young men from Tomsk Region with personality disorders, who had checked by psychiatric military exam et TomskRegionalPsychiatric Hospitalin 1998 and 2003 was carried out. In 2003 to the examination was aimed at 115% of recruits more than in 1998, although the population of the region has not increased. As a result of the examination of mental disorders were found in 65.4% of recruits in 1998 and 92.1% in 2003. Personality disorders were diagnosed in 28.3% of conscripts with mental disorders in 1998 (1 group) and 62.1% in 2003 (2 group), and the clinical pattern of these disorders was significantly different between. The follow-up study revealed, that 5 years later after psychiatric military exam only 21.9% patients from first group and 6.0% from second were under psychiatric supervision.
For the purpose of a comparative estimation of efficiency treatment of a tuberculosis in children of early age, 272 cases for two ten years' periods (2001—2010, 1985—1994) were analyzed. In connection with negative shifts in an epidemiological picture of a tuberculosis last decade (growth of widespread forms of a tuberculosis is more often, at a source of infection were registered the bacterioexcretion and drug resistance), at tuberculosis treatment has increased medicinal loading on an organism of the child that promoted development of collateral reactions to antitubercular preparations and less often it was possible to reach full resolution of specific changes.
There are represented results of 253 tuberculosis lung patients (main group) and with acquiring (comparison group) multidrag resistance in Republicof Buryatia. Index of sputum clean negativations is higher in main group — 77.8%, in comparison group — 44.4% (р < 0.05). Index of conversion sputum clean in main group — 78%, in comparison group — 41.9% (р < 0.01). «Сuring» is registerated in firsting XDR patients group and composes 26% against 10.8% in comparison group (р < 0.01). Initial healing — «unsuccessful treatment» is noted only in group with seconding XDR — 5 patients (2.5%). Index of breaking off treatment in comparison group — 28.1%, in main group 18% patients. Deathing initial from tuberculosis is registerated only in acquiring XDR patients group — 21 (10.3%) desires. Conducting analysis lets to suppose that modern appointment of correct therapy regime for XDR patients furthers to high treatment efficacy achievement.
At present, medical attention is paid to the quality of life for patients. The dependence between the quality of life and the course of the disease, personality patients characteristics. In the conducted study involved 139 patients with cirrhosis of the liver. With the help of questionnaires, self-administered by patients, assessed the quality of life depending on the tone of the autonomic nervous system. As a result of supervision it is established that the vegetative disbalance at sick of a cirrhosis with prevalence of parasympathetic influences worsens indicators of quality of life in spheres of vigor, painful sensations and emotional reactions.

Cortex biopotentials of mother and baby have been measured at their physical fields shielded by a folgoizolon screen. The used screened box decreased the infrared field испытуемых by 95% and the acoustic field by 20%. A possible reaction in the form of decrease of alpha rhythm in the sensory cortex was not observed. The dynamics of the EEG amplitude, power, and rhythm frequency during the tests had individual character and was independent of the presence or absence of a screen. The result obtained may be explained by the fact that the 20% decrease of acoustic signals is insufficient for the deprivation reaction of mother and baby для реакции or other sensory modalities are important for them.
The results of research the cytokine-secretion activity of mononuclear leukocytes, apoptosis and proliferation of lymphocytes for patients with infiltrative pulmonary tuberculosis has been presented in this article. It has been showed that patients with drug-resistant pulmonary tuberculosis had more evident of damages, particularly significant depression of proliferation of lymphocytes, decreased number of cells expressing CD95-receptor as well as disturbance the secretion of immunoregulatory cytokines.
When studying the influence of TB treatment regime on life quality among 55 patients with drug sensitive and drug resistant tuberculosis, the physical functioning limitations, associated with the emotional health, decrease in vitality, and high index of bodily pain was observed among the patients with drug resistant tuberculosis. The life quality continued to decline along with the prolongation of TB treatment, the psychological component was mostly responsive to the therapy.
Introduced for the purpose of increase of rendering of quality of medical care in to service of the Kemerovo region the computer program «Monitoring of TB patients with drug-resistance» in a complex with other programs provides control over selection on treatment, completeness and frequency rate of inspection, adequacy and efficiency of treatment TB patients with drug-resistance, work of the specialized medical commissions.
The comparative analysis of features of clinical symptoms, spectrum and the importance of burdening medico-social factors of a tuberculosis of lungs with primary MDR М. tuberculosis in comparison with a drug sensitive variant of disease is carried out. Presence of associations between burdening factors of disease and MDR М. tuberculosis is proved. We defined degree of risk of detection MBT with MDR at new cases tuberculosis of lungs with narcotism, a tobacco smoking, a syndrome of alcoholic abuse, stay in imprisonment places, a diabetes of 2nd type, hepatitises B, C, В + С.
For the purpose of revealing predictors failure outcome of antitubercular therapy at patients suffering from multidrug resistant tuberculosis we have executed comparative analysis of clinical, laboratory, bacteriological and radiological data in the conditions of DOTS-PLUS program. The received results have shown that predictors were social, medical and biologic factors, according which we can forecast treatment’s outcomes at initial stage.
