NTERDISCIPLINARY BASIC RESEARCH IN MEDICINE
Object of research: to study the interrelation of activity of systemic inflammatory response and quality of life (QOL) and evidence of metabolic disorders in hypertensive patients with metabolic syndrome (MS).
Material and methods. We carried out a study of 86 patients with hypertensive disease of II stage in combination with MS and 18 volunteers, who formed the control group. Along with a complete clinical, laboratory and instrumental examination, taken in a specialized cardiology clinic, the study of QOL using the MOS SF-36 questionnaire was carried out, as well as the research of activity of systemic inflammato-ry response.
Results. We determined the reverse correlative interrelation of QOL indicators with all MS components, such as abdominal obesity, hyperglycemia, hypertriglyceridemia, the degree of arterial hypertension, but also with the level of markers of systemic inflammation (C-reactive protein, neopterin and fibrinogen), hyperinsulinemia and hyperleptinemia. It was found out that physical functioning (PF) has the strongest interrelations with the maximum number of clinical and laboratory indicators of MS and all the studied markers of systemic inflammation.
Study the relation between the characteristics of muscle coordination, balance, and - vegetative-vascular balance on dancers. The dependence of balance and coordination abilities of the nature of the muscle and, in turn, from the vegetative-vascular support activities. The method of correlation analysis were confirmed by the relationship of coordination abilities with the characteristics of the bioelectrical activity of the muscles and vegetative-vascular support. All the above allows us to consider the formation of skills in ballroom dancing as a single functional system, in which the sense of balance and coordination abilities that determine the skill level of athletes associated with the information field of visual and vestibular reception, to the organization of the contractile activity of muscles and vegetative-vascular maintenance activities.
35 children with extremities’ long bones fractures of the different localization were examined. The medium age (10.5 ± 2.0) years. The control group consisted of 35 healthy children: without pathology of the supporting-motor apparatus, medium age 10.5 ± 3.0. The single study of the systemic and ionized calci-um, phosphorus, magnesium and alkaline concentration and phosphatase’s activity in the blood serum was realized using automatic analyzers «Cobas 6000 SWA» (Roche Diagnostics, Switzerland).
The forming and bone resorption process activity was appreciated according to the P1NP (N-terminal propeptide of type I collagen) content in the blood serum, osteocalcin аnd β-СrossLaps (β-isomerized carboxy-terminal cross-linking region of type I collagen) on the modular platform «Cobas 6000 SWA» (Roche Diagnostics,Switzerland). The mineral turnover’s regulation was determined by parathyroid hor-mone, thyrocalcitonin and 25(ОН)-D3 in the blood serum. The growth process activity was determined by content of the somatotropic hormone in the blood serum. Osteocalcin and somatotropic hormone de-termination was done by analyzer «Immulite One» (USА).
The statistical processing of the obtained results was carried out with application program package Statistica 6.0 for Windows. The distribution normalcy control was performed using the Kolmogorov-Smirnov's test. Data were presented as Ме (LQ–UQ), where Ме is median, (LQ–UQ) is interquartile dis-persion. To reveal the differences between groups concerning quantitative readings the Mann–Whitney and Craskell–Wallace tests were used. The disparities were counted as statistically significant in р < 0.05. The correlation analysis was carried out by means of Spearman criterion.
Children with extremities’ long bones fractures demonstrated the decrease of the bony tissue synthesis markers P1NP and calcium ions in the blood serum that speaks for the relative predominance of the resorptive activity of the bony tissue against the hypocalcaemia. There was the increase of the parathyroid hormone and somatotropic hormone levels in the blood serum in children with fractures on the back of the absence of the correlation relationship between these values and bony markers that seems to be the return of the disorders of the endocrine control of the bone-mineral turnover.
The deceleration of the bony tissue synthesis may be connected with hypocalcaemia conditioned by vitamin D deficit received for most of the examined children. On the other side, rickets D is usually the cause of the secondary hyperparathyroidism that can lead to the bony resorption elevation as a whole, osteomalacia progressing and bone mass decrease and increase of the bone’s fracture risk. The up-to-date diagnostics, treatment and prophylaxis of the vitamin D deficit states in children allow to decrease the osteopenia frequency and to reveal the tendency to the fractures.
The work is devoted to the study of atrial natriuretic peptide (ANP) and its precursors as one of possible predictors of left ventricular repeated remodeling in patients with ischemic cardiomyopathy (ICMP) in the follow-up (postoperative period). Objects of the study are venous blood and intraoperative samples of right atrial auricle. Precursors of NUP were identified in blood serum by the immuno-fluorescence test. Ultrathin slices of auricle biopsy samples were examined with electronmicroscopy with the use of image processing ImageJ software (program). The concentration of NUP precursors in blood plasma and relation of minimal diameter of secretory granules to maximal one in right auricle cardiomyocytes reflect pathological processes taking place in the myocardium of patients with ICMP. Ratio of minimal and maximal diameters of secretory granules is a predictor of chronic heart failure progressing.
Clinical course, prognosis and mechanisms of separate forms of heart rhythm disorders in children differ from those in adults. Especially, it refers to new-borns and infants whose conduction system differs by functional and morphologic immaturity. In connection with it, the assessment of natural history of heart rhythm disorders, occurred in a perinatal period, and determination of risk factors of arrhythmia appearance in infants are of some interest. 88 newborns took part in the study. The patients were involved by continuous sampling technique. Risk factors, occurred in a perinatal period and potentially influenced on development of heart rhythm disorders, were assessed. In our study we took biological, gynecologic and obstetric history, data of gestation and delivery course, early and late neonatal period, early infancy, Echo, neurosonography, Holter monitoring with determination of heart rhythm variability, and determined thyroid hormonal status. Maximum specific gravity had extrasystoles – 32.4% – in the structure of idiopathic arrhythmias in infants. Heart rhythm disorders with natural history were kept at six months of life only in 5,4% of children. Persistence of arrhythmias was marked during one year only for WPW syndrome. Heart rhythm disorders are often marked significantly in children whose mothers had acute respiratory disease during the pregnancy, or if the children were born from the first pregnancy, had the signs of central nervous system damage syndromes in an early perinatal period (arrest, intracranial hypertension, convulsive disorder). Disorders of autonomic imbalance of cardiac function and peculiarities of hemodynamics of pulmonary circulation contribute significantly into appearance and persistence of all types of arrhythmias. On the whole, the prognosis of heart rhythm disorders, occurred in the perinatal period, without organic and structural changes of myocardium is favorable. The exclusion can be made for congenital atrioventricular block and some types of supraventricular tachycardia which are not responded to treatment.
Aim. Study of the exchange of liver and blood plasma biopolymers of alloxan diabetic rats.
Materials and Methods. Diabetes mellitus was modeled in rats by single subcutaneous injection of alloxan tetrahydrate (170 mg per100 gbody weight). Blood glucose, glycosylated hemoglobin were controlled and morphometric study of the pancreas was carried out for the verification of the model. A month later, concentration of glycosaminoglycans, free hydroxyproline and the level of hyaluronidase and collagenolytic activity in plasma were determined. The total concentration of collagen, glycosaminoglycans, and their fractions, the level of hyaluronidase and collagenolytic activity in rat liver homogenate were measured.
Results. The level of all the parameters of interest in the liver and blood plasma increased on 30 day after alloxan injection, the accumulation of glycosaminoglycans in the liver occurred mainly due to unsulfonated fraction.
Conclusion. The development of experimental diabetes in rats is accompanied by activation of both decay processes and synthesis of biopolymers studied. Accumulation of total collagen and glycosaminoglycans was observed in rats’ liver, which probably lead to the fibrosis changes in it.
The retrospective study was designed to identify association of premature rupture of the fetal membranes (PROM) with carrying polymorphisms in genes encoding folate metabolism and hemostasis in 717 women. More than one hundred potential predictors were analyzed including carriage of thrombogenic genes polymorphisms and genes encoding folate metabolism: FV[Arg506Gln], F II [20210 G/A], MTHFR [Ala222Val], (PAI-I)[-675 5G/4G]. Study revealed that plasminogen activator ingibitor-1 gene polymorphism increases significantly the risk of premature rupture of the fetal membranes in term pregnancy (PROM): heterozygous plasminogen activator ingibitor-1 gene polymorphism is associated with 3.6-fold (95% CI 2.4–5.4; p < 0.001), homozygous plasminogen activator ingibitor-1 gene polymorphism – with 1.7-fold (95% CI 1.1–2.6; p = 0.01) risk rise of PROM.
Continuous sampling method using aquestionnaireHospitalAnxiety and Depression Scale (GSHTD) held survey 250 patients with acute coronary syndrome (ACS) admitted to the emergency Department of cardiologyInstituteofCardiology, SB of RAMS. Of these, 85 people (34%) were detected symptoms of anxiety and depression in 165 people (66%), anxiety and depressive symptoms were found. In a pilot randomized prospective comparative study included 46 people. Against the background of the basic treatment of ACS, 23 patients received an additional agomelatine. Purpose: To study the distribution of genotypes and alleles tumor necrosis factor-α G–308A, interleukin1β C–511T, interleukin-10 G–1082A in patients with slowlyresolved pneumonia and their association with clinical and radiologicalfeatures of the disease.
Materials and Methods: We investigated 89 patients with community-acquired pneumonia, of which 37 people had a slowlyresolving course of the disease.The genotype distribution of studied polymorpisms corresponded the Hardy–Weinberg equilibrium.
Results: We determined that the tumor necrosis factor-α G–308A and interleukin-10 G–1082A gene polymorphisms were associated with the long course of pneumonia. Slow resolution of the disease in patients who are carriers of GA and AA genotypes of tumor necrosis factor-α contributed to a severe course. The intensity of inflammation in these patients was reflected in the high rates of erythrocyte sedimentation rate and C-reactive protein. Prolonged duration in patients who are carriers of GG genotype of IL-10 was associated with significant clinical and laboratory manifestations at onset of the disease and more frequent development of pleural effusion. The frequencies of alleles and genotypes of interleukin-1β in observed patients did not differ.
Conclusions: The data can be used for prognosis of slowlyresolved pneumonia
We studied the course of the disease in 114 patients with invasive ductal breast cancer stage T1-4N03M0, held a combined treatment at the Department of General Oncology FGBU Cancer Research Institute of Oncology, SB RAMS in the period from 2000 to 2007. Revealed additional information of importance for the prediction of hematogenous dissemination in patients with different molecular-genetic types of breast cancer. Predict this worst-case flow of tumor on the basis of information about the regional lymph nodes is possible only with triple negative breast cancer, and only in patients who are in a state of menopause.
Registered spirogram and transpulmonary pressure in spontaneous breathing in 10 healthy volunteers and in 10 patients with community-acquired pneumonia. For the first time the work inside the lung-a source of mechanical energy (RVI) was measured by the method of interruption of the air on 0.2 with inhalation and exhalation in the middle of the respiratory cycle. Alveolar pressure exceeding the total nonelastic pressure on the inhale and exhale through self-mechanical activity of the lungs. RVI was determined by the area of a triangle, where one cathetus were breathing capacity and the amount of excess air pressure. The total work of breathing (TWB) was measured by the square of the chart pressurevolume. Total breathing (Рt) was determined by adding TWB and RVI. RVI average was 29.6 (25.0– 43.3)% to Рt in healthy individuals and to 19.4 (0–55.1) % of patients with community-acquired pneumonia. In 2 patients, RVI has not been determined, and 2 were increased significantly. The nature of variations RVI in healthy people and change its degree of severity of community-acquired pneumonia remain unknown
The weekly in vitro degradation of fibrous-porous non-woven polylactide scaffolds made by aerodynamic formation in a turbulent gas flow has been studied with 37 °С in model RPMI-1640 medium imitated body fluid of organism. Lactate monomers released into solution exponentially and reached slowly a maximum value the end of the observation (5th week of dissolution). At the same time, reducing the concentrations of calcium and inorganic phosphorus ions in solutions contacted with tested samples (10×10×1 mm2) testified about chemical elements adsorption on artificial material. Ions exchange with biological fluids may be a basis of regulated bioactivity of fibrous-porous non-woven biodegradable material in application to intercellular matrix bioengineering for regenerative medicine
Imperfect decrease of systolic and diastolic pressure at night is observed according to 24-hour blood pres-sure monitoring at patients with metabolic syndrome and senilism. According to average daily pulse arte-rial pressure 55.0 (51.0; 60.0) mm. of mercury column the patients with senilism are referred to the risk group to appear cardiovascular complications
The problem of the bacterial cystitis treatment continues to be relevant among the urologists all over the world due to its prevalence rate and the complexity in its treatment. The goal of research: to study experimentally the influence of synthetic glucocorticoidtriamcinоlon on the inflammatory mucosa of the urinary bladder.
Material. The experimental study was conducted on the outbred white reproductive rats-females. The main group of the animals (40 rats) was exposed to simulation of the inflammatory mucosa of the urinary bladder. Triamcinоlon instillations (0.3 ml) were performed on 20 rats. In the other group of rats the triamcinоlon instillation was added with intramuscular injection of antibiotics. There were two compari-son groups of rats with the simulated inflammatory mucosa of the urinary bladder. The first (20 rats) re-ceived only intramuscular antibiotics injections and the second (20 rats) received no treatment. The con-trol group of animals (10 rats) received instillations of intact 0.9% NaCl.
Methods. The bladder wall ultrastructure was analyzed by the method of transmission electronic microscopy.
Rresults. According to the research data, decongestion of the swelling enchasing mass was observed after 5 days of triamcinоlon instillations both with antibiotics and without it in comparison with the inflammatory process without any treatment and with the antibiotics injections only. After 10 days this result in the main group was the same as in the control group. Infiltrating cells density in the main group after 7 days was obviously lower than in the comparison groups. The triamcinоlon injections induce neovasculogenesis and fibrogenesis, improve hemorheology in the vascular plexus of the urinary bladder mucosa. The results come up to a high level of statistical significance.
Summary. The experimental data prove the safety of intravesical triamcialon instillation and its therapeutic effectiveness.
INNOVATIVE MEDICAL TECHNOLOGIES
The integral and regional values tissues of non-elastic resistance lungs (NERLi, NERLr) in 17 patients with bronchial asthma (BA), 25 – chronic obstructive pulmonary disease (COPD) and 30 healthy volunteers are investigated by means of simultaneous recording of zonal reogrammes of ventilation and transpulmonal pressure.
The increase of NERLi in patients with BA is accompanied by the increase of the NERLr values inhaling in the lower zones, and exhale in the middle and lower areas of both lungs. Significant increase of NERLi In COPD accompanied by the increase of the NERLr values on the exhalation in the middle and lower areas of the right lung. And values of NERLr in BA almost 2.5 times higher than the similar values in patients with COPD. Apparently, in BA is due to an increase in the permeability of the alveolar-capillary membrane in connection with an immune inflammation, and in COPD – effect of other factors, among which the actual tissue friction and reducing of mechanical activity light to overcome the total resistance.
Research aim was definition of prevailing coping strategies of patients with inflammatory bowel diseases of in comparison with healthy people. 49 patients with ulcer colitis are surveyed. Coping strategies were defined by means of E.Heim's test. Patients with ulcerative colitis more often than healthy used to maladaptive coping strategies in the emotional and cognitive sphere that can testify to violation of psychological adaptation.
Objective: to compare the therapeutic possibility of the decompressiveviedecompressive with stabilization surgeries using the standard posterior and anterior retroperitoneal approaches in patients with combination of inherent and obtaining lateral stenosis and to demonstrate the adequacy of using. At the main group we removed the herniated disc with stabilization using anterior and posterior approaches – 82 patients. The control group was treated by standard microdiscectomy – 40 patients. More excellent and good results were in the main group where decompression was combined with stabilization, and at the same group were less negative results. The decompressive-stabilizing surgery with anterior interbody fusion is a pathogenetic and technically adequate treatment for combined lateral stenosis.
The task of this research was a study of the functional state of the biliary system and gallbladder bile composion in patients with the inflammatory bowel diseases (ulcerous colitis and Crohn 's disease).
Summary: The results showed a reduction in motor activity of the biliary tract and an increase the lithogenicity of the gallbladder bile in patients with average and severe degree of severity of ulcerative colitis and with Crohn's disease. All this can increased the risk of gallstone formation in these patients
Early postoperative rehabilitation of patients with cholelithiasis is aimed at improving the function of bile secretion, adaptability and normalization of psycho-vegetative state body, which in the aggregate prevents progression of the disease and reduces the risk of postcholecystectomy violations. Use in rehabilitation activities fresh mineral water, magnetic-laser and EHF-therapy allows to receive significant improvement of the studied parameters in a significant improvement and normalization of clinical and laboratory indicators, increase adaptive capacity and normalization of psychoemotional and vegetative status of the organism. It is provides immediate high efficiency of the activities (94.7%) of early postoperative rehabilitation of patients with cholelithiasis, which reduces the risk of the development of postcholecystectomy violations and prevents progression of the disease.
The aim of work is valuation of the effectiveness of autologous lymphocytes activated by complex of cytokines to prevent erysipelas of patients with after breast cancer syndrome.
The estimate of influence of activated autologous lymphocytes to erysipelas. The treatment results of 128 patients with secondary lymphedema of the upper limb II–III stage after curative treatment of breast cancer are included in the work.
Activated roncoleukin auto lymphocytes injections were performed in the forearm of the affected limb to the main group’s patients.
17.4% faces relapses were recorded in the comparison group. while the group with the usage of activated roncoleukin autologous lymphocytes had achieved significant reductions in recurrent of erysipelas up to 5%.
LITERATURE REVIEW
In the article, problematic questions of diagnostics and therapy of non-psychotic mental disorders (NPMD) in female patients of climacteric age with hysterical symptom complex are considered. Efficacy of psychotherapy (PT) in NPMD, hypnopsychotherapy in hysterical states: hysterical neurosis, neurasthenia and obsessive-compulsive neurosis is indicated. In treatment of NPMD, PT by creative selfexpression is successfully used. It is highlighted that PT forms conscious-critical attitude of patients toward themselves. Combination of PT with physiotherapy in hysterical conversional symptoms has been described. In hysterical manifestations neuroleptics are recommended, in neurotic depressions – antidepressants of mild action. In severe hysterical state, psychopharmacotherapy (PPhT) with tranquilizers and neuroleptics is applied on long-term basis. Stable recovery in dissociative and hysterical disorders has been shown. In vegetovascular disorders in structure of climacteric syndrome (CS) vinpocetine, in psychoemotional manifestations phenibut was administered. In therapy of hysterical neurosis, “minor neuroleptics”, hypnosuggestive therapy, social rehabilitation were applied. Effective group PT of psychogenically conditioned disorders in asthenicand anxiety-depressive symptoms is effective. Complex therapy of NPMD in hysterical and asthenic neurosis, obsessive-compulsive neurosis has been suggested. Organization of specialized preventive examinations for early revealing of persons with personality pathology is based. Efficacy of a number of medications in periand post-menopause – SSRIs and gabapentin, during menopause paroxetine, in depressions of non-psychotic level – pyrazidol, coaxil, in neurotic hypochondriasis sulpiride and quetiapine, diazepam, in climacteric vegetative and mental disorders hormone replacement therapy (HRT), hormonal therapy, PPhT and PT, in neurovegetative symptoms of CS – antidepressants, in psychovegetative syndromes – SSRIs, in dysthymic disorders – tianeptine (coaxil), in involutional hysteria – psychotropic medications and medications of neurometabolic action, in psychoemotional disorders of mild and moderate degree – deprim, in depressive disorders of mild and mean severity – valdoxan has been shown. In climacteric depression, hormonal therapy stops climacteric symptoms.
Due to the fact that nowadays mechanisms of syntropy of pathological conditions and nosological units, united within the metabolic syndrome, remain unclear, the scientific review attempts to summarize data on the role of fatty tissue inflammation in pathogenesis of this symptom complex. The results of recent major foreign studies on evaluation of pro-inflammatory activity of adipocytes and macrophages of the fatty tissue, as well as the data on peculiarities of their interactions in abdominal obesity, which is the main component of the metabolic syndrome, were analyzed. Studing pathogenesis of fatty tissue inflammation from the perspective of evaluation of disorders in cell cooperation will allow to more deeply understand cellular and molecular mechanisms of this process as well as open new avenues for developing new pathogenetically justified approaches to metabolic syndrome treatment.
A review of modern methods of magnetic resonance imaging (MRI) and emission tomography (singlephoton emission and positron emission computer tomography – SPECT and PET) as toos for diagnosis and prognosis of myocardial ischaemic damage, in particular in coronary revascularization. The definition of term “myocardial viability” is discussed. It has been shown that the integrity of blood-tissue barrier between myocardium and microcirculatory vessels is the most sensitive marker of tissue viability and of functional integrity of myocardium. It’s evaluation by means of contrast-enhanced MRI of myocardium is the most available and most precise technique of diagnosis and prognosis both in patients with postinfarction myocardiosclerosis and in patients with coronary disease without myocardial infarction. It is proposed that in the nearest future the combination of MR-coronarography and contrast-enhanced MRI of myocardium will provide a possibility to obtain the full set of data necessary for planning of endovascular and surgical treatment of various forms of coronary heart disease. PET and SPECT techniques currently are of some essential interest for pathophysiologic research of coronary ishaemia in clinical and experimental studies as well as for qualitative visual studies of pharmacokinetics.
The article is a review of current literature on a content of proteins, peptides and amino acids in human exhaled breath. The results of proteomics and metabolomics applying for selective detection of individual proteins, peptides and amino acids are described. The study of exhaled breath condensate and exhaled endogenous particles contained lung proteins are considered. The peculiarities of protein, peptide and amino acid content in exhaled breath at various respiratory diseases are described. It is shown that the detectable substances may be specific markers of individual diseases.
МОЛОДЫЕ ИССЛЕДОВАТЕЛИ – МЕДИЦИНЕ
In order to evaluate cytogenetic effect of nanocomposite materials used in traumatology and orthopedics an investigation of chromosomal aberration ins in the culture of lymphocytes short-term contacted with artificial material (nanostructured titanium samples of implants bearing bilateral calcium phosphate coating) in patients with dysplasia of connective tissue (DCT) and osteogenesis imperfecta (OI) has been conducted. A reduction of percent of aberrant cells having single or/and paired chromosomal breaks as compared with spontaneous mutagenesis (without model samples) was revealed under introduced biocompatible composites into blood cell cultures of patients with DCT or OI. Analysis of frequency and spectrum of chromosomal aberrations in blood cells can be considered as potential prognostic criterion and a predictor of implant’s efficacy in the surgical treatment of DCT and OI complications.
DISCUSSIONS
Results of research of biomechanics of breathing at patients with progressing muscular dystrophy (PMD) and their interpretation are unique. Compared indicators of ventilating function of lungs and indicators of mechanics of breathing at 31 patients with PMD to 1–2 extent of motive frustration and 17 patients with 3–4 extent of motive frustration. In both groups of sick MVL and OFV-1 are lowered to the same extent, bronchial resistance isn’t increased. In the 2nd group the reserve volume of an expiratory is reduced for the account decrease in force of respiratory muscles is decreased. Elastic draft of lungs is reduced, the coefficient of functional activity of lungs is increased. At spontaneous breath the tensile properties of lungs are lowered, increased elastic fraction of work of breathing. The general nonelastic resistance of lungs is on the average equally increased in both groups at the expense of increase of tissue friction. At 12 sick PMD the abnormal respiratory loop came to light: completely I was absent at 8 patients and at 4 there was no inspiratory or expiratory part of a loop. Changes of indicators of mechanics of breath are the in generally functionally, and are considered as manifestation of compensatory strengthening of function of an intra pulmonary source of mechanical energy.
CLINICAL CASE
Two clinical cases that demonstrate the difficulties diagnosing the causes of shortness of breath, coupled with anxiety, are analyzed. From the standpoint of modern neurophysiology it explains the relationship of these conditions.
