Abstract

Aim. The influence of dry extracts of Saussurea controversa DC. and Filipendula ulmaria (L.) Maxim. in experimental osteomyelitis with the use of сefazolin in rats.

Materials and methods. After a course of therapy extracts at a dose of 100 mg/kg on the background of antibiotic therapy observed localization of the inflammatory process, whereas animals receiving only Cefazolin becomes diffuse inflammation. In the conditions of complex therapy the level of diene conjugates significantly reduces in the serum and the level of TBA-active products, total lipids and lysophosphatidylcholine reduces in the muscle tissue of the affected area. In the application of extracts of S. controversa returns to normal level of diacylglycerides and phosphatidylcholine, and the application of the extract of F. ulmaria – free cholesterol in comparison with the group of animals treated only with сefazolin.

Results. In the application of water-ethanol extract of S. controversa with simultaneous intramuscular сefazolin marked decrease in the activity of acid and alkaline phosphatase that can enhance reparative processes in the bone tissue.

Abstract

Objective. To assess risk factors оf pedicle screw instability after decompression and fusion in patients with degenerative lumbar spine disease.

Materials and methods. This is a prospective non-randomized study, the results of 130 surgical interventions with pedicle instrumentation in patients with degenerative lumbar spine diseases were studied. Minimal follow up period accounts for 18 months. Before intervention computed tomography (CT) was applied and cancellous bone radiodensity was measured.

Patients were treated with nerve root decompression and pedicle screw fixation stand-alone or with lumbar interbody fusion. During follow-up period computed tomography was applied and cases with evidence of pedicle screw loosening were detected. Radiodensity of cancellous bone, extension of fixation, extensiveness of decompression, application of interbody fusion, incomplete vertebra body reduction and heterogeneity of studied group were taken into account as potential risk factors of implant instability development. The correlation between pedicle screws loosening rate and mentioned predictors was estimated using logistic regression analysis.

Results. Radiodensity of vertebra body cancellous bone getting decreased, the increase number of fixed levels and extensiveness of facet joints and ligaments resection are associated with the increased risk of a screw loosening development. Laminectomy, interbody fusion, altered biomechanics associated with incomplete vertebra body reduction and L5–S1 segment included into fixed zone did not have a significant influence on pedicle screws loosening rate. Bias related to heterogeneity of studied group was also insignificant. Goodness-of-fit of estimated general logistic regression model: № 2 = 67,57851; p < 0,0001. This model classified correctly 81,5% cases with sensitivity and specificity of 77,4% and 85,3% respectively.

Conclusion. Radiodensity of a vertebra cancellous bone, number of fixed levels and extensiveness of facet joints and ligaments resection during decompression are significant predictors for pedicle screws loosening development. The detected risk factors should be taken into account in preoperative planning especially in elderly patients.

Abstract

The aim of this study was to investigate the outbreak of aseptic meningitis in theNovosibirskregion in 2008–2009. We studied genetic diversity and molecular-epidemiological characteristics of human enteroviruses that caused aseptic meningitis. Materials and metods. In the present study we investigated samples of cerebrospinal fluid from 199 patients with a diagnosis “aseptic meningitis”, based on the clinical characteristics of the disease (headache, stiff neck, fever, nausea, vomiting), and confirmed by laboratory tests of spinal fluid (lymphocyte cell count > 10 cells/ml).

All samples were tested by PCR for RNA of Enterovirus and Flavivirus and DNA Myc. tuberculosis, Borrelia spp., Neisseria spp. In the samples there were not found RNA Flavivirus, DNA Myc. tuberculosis, Borrelia spp., Neisseria spp., but in 73 samples (37%) was identified RNA enterovirus (EV). Determination of nucleotide sequences of 5’UTR and VP1-region of EV revealed that they belong to the following genotypes: the highest percentage was presented by genotype ECHO 30 (62%); another genotypes were Cox A2 (8%), Cox A4 (5%), Cox A14 (3%), Cox A16 (5%), Cox B5 (8%), ECHO 6 (3%), ECHO 9 (3% ) and ECHO 25 (3%). In 2008 most of the EV that caused the symptoms of aseptic meningitis belonged to genotype ECHO 30 (76%). In 2009 the clinical specimens containing genotype ECHO 30 were not found, but the largest percentage of EV belonged to genotypes Cox A2 (33%) and Cox A4 (22%). Thus, in 2008 we recorded outbreak of aseptic meningitis, the major etiological factor was enterovirus ECHO 30. And the rise of the incidence of aseptic meningitis in 2009 is related to the circulation of new genotypes of EV. The investigated strains were deposited in an international database GenBank under accession numbers KP258231-KP258235, HM559584.

Abstract

The aim of research is to study the state of circadian arterial tension profile in the patients with bronchial asthma and hypertensive disease as comorbid disease. Materials and methods. The research has been performed at 76 patients with bronchial asthma and hypertensive disease as comorbid disease (the main group) and 52 patients with hypertensive disease as the comparison group. The groups were comparable with respect to the gender and age sign. Investigation was performed in the period of clinical remission. The main method used in this research was the investigation of day arterial tension profile in the time of its monitoring.

Results. It has been revealed the presence of frequent and expressed change from the side of the studied indexes, especially in the patients with comorbid pathology, which were characterized by more frequent and more significant disorders from the side of diastolic blood pressure, especially at night in combination with more considerable and more rapid rise in early morning hours. The day arterial tension profile was characterized either with insufficient decline of arterial pressure at night or, opposite, with its sharp decrease. Specified disorders were increased as far as heaving of main and comorbid diseases, presence of disorders from the side of lipid exchange were not related to the phase of bronchial asthma (remission, exacerbation) and level of its flow control.

Abstract

ABSTRACT Aim of the study is to develop the diagnostic checklist based on the incidence and significance analysis of the external features of the dysplasia of the connective tissue (DCT) for its screening and assessment of its prevalence in the adolescents’ population.

Materials and methods.110 adolescents (aged from 10 to 14 years) were examined, 81 of them had the dysplasia of the connective tissue (the first group). The second group was presented by 29 adolescents without DCT. The presence and degree of manifestation of DCT were established according to the criterions of Kadurina T. I., Gorbunova V.N. (2009), Т. Milkovska–Dimitrova (1987). There were the physical, laboratory and instrumental examination. Statistical processing of the results was carried out using Statistica 6.0 program and IBM SPSS Statistics 20.0. The diagnostic efficiency of the developed checklist was appreciated by means of the receiver operating characteristic curve (ROC curve).

Results. The frequent occurrence of the DCT’s external features in adolescents was identified. The diagnostic checklist was developed. It was consisted of 12 items with illustrations, allowing readily identification of the dysplastic phenotype presence with diagnostic sensitivity 81.5% and specificity 79.3%. This checklist can be recommended as for the population studies, as for screening about DCT in adolescents during the primary outpatient pediatrician attendance. In the positive result of the screening it is necessary the further checkup to detect the organ changes and diagnosis updating.

Abstract

Aim. We studied via low vacuum scanning electron microscopy the effects of a surgical laser exposure to adrenal glands and results of regeneration processes after. Materials and methods.Purpose of this work is modeling of effects of the removal with a surgical laser a pathological focus in the adrenal glands. For this Wistar male rat (n = 19) adrenal glands were researched without the laser exposure, immediately after it and 1 month later.

Results. Immediately after exposure occurs laser ablation crater with rough edges and melted surface penetrated by equidistant pores, which are footprints of blood vessels. Beneath of the surface are numerous vaporizationbubbles. Around the crater, the surface wrinkles and sags due to decreased ability to retain water. 1 month after the laser damage, the affected area tightened by a scar. Its coarse bundles of collagen fibers braid shapeless lumps of coal and caverns. Tissues with normal appearance are close to the scar, both outside and inside of the organ. The wrinkling and the sagging are absent. The undamaged organ part has retained the previous shape, without hypertrophies. The damaged part has shrunk. The nature of the regeneration processes indicates a low probability of a relapse after the destruction of a pathological focus via the surgical laser exposure.

Abstract

The flowers of the pot marigold (Calendula officinalis L.) are part of combination herbal medicinal preparations which applied in the treatment of diseases of the hepatobiliary system, gastrointestinal tract, diseases of kidneys and the urinary tract, such as «Fitonephrol» («Urological tea»). Calendula flowers make a contribution in the total therapeutic activity of «Urological tea» as a component which has anti-inflammatory and reparative properties. As is widely known that phytopharmaceuticals usually combine the variety of therapeutic activity and softness of pharmacological effects along with the absence of a significant number of side effects and contraindications which is extremely important in the treatment of chronic pathologies. The main purpose was to study diuretic activity of phytopreparations of pot marigold.

Materials and methods. Investigations of discovering of diuretic action of pot marigold’s phytopreparations were carried out on white rats of both sexes weighing 200–220 g (8 sets of experiments). Each experimental group consisted of ten animals. The effect of pot marigold medicines at a dose of 50 mg/kg was carried out background the administered intragastrically at 3% aqueous load of 4 and 24 hours of the experiment. There are such drugs as a subjects to examination: calendula tincture (1 : 10) (industrial production), calendula tincture (1 : 5) and calendula liquid extract (1 : 2) of 70% alcohol, obtained at the Department of Pharmacognosy with Botany and Bases of Phytotherapy, at a dose of 50 mg/kg. Animals were housed in the vivarium on a standard diet with free access to water. The day before the experiment the rats received intragastric water load in the amount of 3% of body weight. On the day of the experiment the animals of the control group was administered water-alcohol load and experienced – intragastrically drug dose of 50 mg/kg in an identical volume of water. As a control was used 70% alcohol. Animals were placed in metabolic cages on day. Collected urine of rats were examined. Determined excretion of water. The concentration of sodium and potassium was detected by flame photometry (mark PAG-1) for 4 and 24 hours of the experiment. The concentration of creatinine was detected by colorimetric method (mark CPHC-3) for 4 and 24 hours of the experiment. The results of experiments were performed statistical on the Mann-Whitney test. Experiments were carried out with all the necessary ethical requirements.

Main results. It was found that among the studied drugs on 4 hours of the experiment the liquid extract indicate diuretic properties. Tincture (1 : 5) showed expressed diuretic activity, saluretic and keratin properties. Tincture (1 : 10) had tendency to increase diuresis. Such effects decreased within the day.

Conclusion. Preparations of marigold flowers in the form of tinctures (1 : 5) and the liquid extract (1 : 2) 70% alcohol at a single intragastric administration to rats at a dose of 50 μl/kg reveal a marked diuretic effect, significantly increasing the amount of urine, excretion electrolytes and creatinine relative to the control. In consideration of the fact that preparations of Calendula flowers have antiseptic and anti-inflammatory properties, the results have a great interest in terms of the complex therapy of acute and chronic kidney and urinary tract diseases.

Abstract

The purpose of this study was to research the psychometrical parameters and the clinical-dynamic features of patients with schizophrenia in depending on their constitutionally-morphological type.

Material and methods. The examination of 86 patients with schizophrenia who are hospitalized at Department of endogenous mental disorders of Mental Health Research Institute was conducted. The study included patients of age from 18 to 65 years old with prescription of catamnesis at least 1 year and whose state at the time of examination corresponded to the diagnostic criteria for schizophrenia from the ICD-10 (International Classification of Diseases 10th Revision). The examination included filling in a modified version of Basic Card of Standardized Description of the Patient with Schizophrenia and his Relative, Positive and Negative Syndrome Scale (PANSS), Clinical Global Impression Scale (CGI). In addition to that patients were examined using anthropometrical method by V.V. Bunak. Statistical significance of differences in the data was evaluated using the Kruskal – Wallis test by ranks, the Mann – Whitney U-test, and Pearson’s chi-squared test. The correlation analysis was performed by the Spearman’s rank correlation test. The K-Means Cluster Analysis was used to determine of the qualitative interrelation and conjugation between the clinical characteristics of the examined.

The results of the study revealed that many parameters of the functioning in these patient groups have statistically significant differences. Use of the K-Means Cluster Analysis was allowed to reveal the internal structure of the data, to group the individual observations according to their degree of similarity.

Conclusions. The obtained data leads to the assumption that the constitutionally-morphological type of patients has an impact on the clinical course of schizophrenia, as well as their adaptation abilities. Asthenic constitutionally-morphological type is a factor of adverse clinical dynamic of schizophrenia and associates with persistent negative disorders and suicidal actions. Overall, this study continues integrative anthropological trend in psychiatry and lays the foundation for further study of the constitutional factors effect on the course of schizophrenia. The obtained results can serve as a model for studying the facade symptoms in general schizophrenia spectrum disorders and be used in the development of individually oriented therapeutic and rehabilitation programs in clinical psychiatry.

Abstract

The aim of this study was to investigate the role of the Na+, K+, 2Cl -transport and potassium conductance, in the implementation of the effects of carbon monoxide in the SMC taenia coli and ureter of guinea pig.

Materials and methods. The study was conducted by the method of double saccharose bridge. Was studied the effects of carbon monoxide donor CORM2 in ureter and smooth muscle of guinea pig of taenia coli, in normal Krebs solution, against inhibitor-bumetanide and potassium channel blocker tetraethylammonium chloride (TEA). Studied the effects of bumetanide in normal Krebs solution and the background of TEA.

Main results. CORM2 causes inhibition of contractile activity of SMC taenia coli and guinea pig ureter. Its effect is weakened by bumetanide and TEA. Bumetanide causes inhibition of contractile activity of SMC taenia coli, and its effect is weakened by TEA.

Conclusions. In this way, it shows that there are tissue-specific mechanisms of interrelation effects CO and ion conductivity.

Abstract

Aim. The article illustrates the way of endothelian – epithepial corneal dystrophy regeneration with intracameral using of authological mononuclear blood cells.

Materials and methods. The series of experiments were made on Wistar rats. The main group of animals (12) was done intracameral injection the authological mononuclear blood cells on the posterior corneal surface after corneal dystrophy simulation. The control animals group (12) was made a tradition treatment.

The results experiment has shown a high effect of mononuclear cells treatment. The number of dystrophy changed epithelial cells reduced in the main group on the third day after treatment start. The proliferation of endothelian was shown on the seventh day after the treatment in the main group. On the fourteenth day the corneal endothelian was restored in the main animals group. The corneal endothelian layer in the control group hasn’t restored on the twenty first day.

Thus, intra cameral injection of authological mononuclear blood cells induces the regeneration of corneal endothelian layer in endothelian – epithepial corneal dystrophy treatment.

Abstract

The article is devoted to the investigation of the possibility of immobilized on the surface of the titanium implantable devices (model plates, “U-shaped” brackets) antimicrobial substances.

Material and methods. The object of research were modeling titanium plates and titanium “U-shaped” brackets for implantation. Ciprofloxacin, hydroxypropylmethylcellulose and methylcellulose has been proposed as materials for the creation of antimicrobial pharmaceutical composition to immobilization. The antimicrobial properties of titanium implantable “U-shaped” brackets with ciprofloxacin was evaluated for potential antimicrobial activity against medically important bacterial (S. aureus, P. aeruginosa, E. coli, E. faecalis and S. pyogenes) using method of diffusion in agar.

 Results. The results showed the possibility of putting and holding а ciprofloxacin in grooves on the modified surface of titanium implants. In the course of the study was developed a pharmaceutical composition, comprising ciprofloxacin and hydroxypropylmethylcellulose. The results showed high antimicrobial activity of pharmaceutical composition with ciprofloxacin against the test organisms (S. aureus, P. aeruginosa, E. coli, E. faecalis and S. pyogenes).

Abstract

Increasing the number of patients with complicated forms of chronic pancreatitis and pancreatic cysts observed in recent decades. Mostly people of working age are susceptible to disease. This makes the issue a social importance.The article presents a modern view of the choice of method of surgical treatment of chronic pancreatitis and cystic optimal terms of therapy, depending on the degree of “maturity” of pancreatic cysts. A detailed analysis of both traditional surgery and advanced minimally invasive treatment for pancreatic cysts is performed in this review of the literature.Emphasis is placed on radical methods of treatment, particularly in the duodenum-preserving operations. Pathogenic study is carried out. The problem of choosing the most radical and at the same time the organ-preserving technique, helping to improve the immediate and long-term results, the quality of life and social and labor rehabilitation, has not lost its relevance. Studies carried out in this area are currently important.

Abstract

The article presents the main stages of the historical development of the temporo-mandibular joint surgery. It was shown the evolution of treatments for diseases and injuries of joints. It summarizes the main work of domestic and foreign authors that influenced the development of surgery of the temporo-mandibular joint.

Abstract

The review presents the opening story, biogenesis and functions of basic groups of human’s small noncoding RNA: microRNA and short interfering RNA. These RNA molecules inhibit gene expression during translation by RNA interference. It was found that microRNA and short interfering RNA circulate in bioliquids and can serve as biomarkers of different human diseases because of its conservative sequences, tissue specificity and resistance to environment factors. The paper considers techniques to study noncoding RNA (cloning, bioinformatics analysis and hybridization methods: northern-blotting, RT-PCR, in situ hybridization, microarray analysis, reporter analysis). Possible noncoding RNA-targeted therapy can suggest delivery microRNA, anti-microRNA, antagomirs, microRNAsponges to target tissue by virus molecules, liposomes or nanoparticles.

Abstract

Aim. The application of method of 3D-visualization and modeling in thoracic oncology is described.

Materials and methods. The block diagram of system of 3D-visualization and modeling consisting of six stages is shown. The reconstructions of anatomic computer 3D-models of rib cage and tumor were performed for the patient with cancer (plasma cell myeloma). The tumor size and its topographic anatomy relatively to neighboring organs, bones and soft tissues were identified.

Results.The obtained data had been used to plan surgical intervention, which was successfully conducted at Thoracic surgery department ofTomskRegionalClinicalHospital.
