Modulating influence of electric impulses, visible light, some microelements is studied in parameters which are near to physio-logic ones on life activity of progenitor and mature cells of blood system in vitro and in situ. Content of granulocyte and fibroblastoid colony-forming units, concentration of colony-stimulating and colony-inhibiting activities, their concentration were determined , myelograms and morphologic signs of myelocariocytes apoptosis and necrosis were scored, saturation by chrom ions of peripheral blood erythrocytes was studied in mice bone marrow. The results showed that cell effects of physical-chemical influ-ences are small specific. Nevertheless, one may perform both positive and negative regulating influence on life activity of stromal precursors in dependence of energy density and stimulus concentration . The effect is due not only by direct but mediated reactions regulating direction and intensity of metabolic intracellular processes. It is suggested that one of target cells capable of change in-crease program of hemopoietic and stromal cells-progenitors of bone marrow in dependence of physical stimulus strength are lym-phocytes.
EXPERIMENTAL AND CLINICAL INVESTIGATIONS
Parameters of breathing biomechanics in healthy persons (n = 20), patients with bronchial asthma (n = 30) and chronic obstruc-tive pulmonary disease (n = 30) are analyzed during electrical stimulation of the diaphragm. Methodology of homeostatic parame-ters searching and their classification is offered. Descriptive and comparative analyses are performed. Homeostatic parameters of biomechanics describing the condition of elastic and non -elastic properties of respiratory system, of respiratory muscles, of general pulmonary hysteresis, breathing regulation are differentiated. Basic homeostatic parameter is the ratio of inspiratory capacity to the lungs elastic recoil. The model of lungs with the biomechanical buffer and retractive-elastic- surfactant complex of lungs is offered. Biomechanical homeostasis idea of respiratory system as ability of an organism to support in dynamics balance normal and patho-logical conditions essentially important for preservation of respiratory system biomechanical parameters in admissible limits is for-mulated.
Experiments performed on white outbred 100 rats showed that light impact with the intensity of 6 000 lx y during 6 hours and radiation in the setting of alloxanic diabetes caused injury of most structural components of retina and significant microcirculatory disturbances in the sclera. Starting with 7 days after radiation, retina damages had focal character which was more evident during radiation in the setting of alloxanic diabetes. However, prolonged injection of ascovertin led to decreased decstruction and improved hemodynamics which was expressed in reliable decreasing focal area by 2.1 times after light impact and by 1.4 times after radiation in the setting of alloxanic diabetes.
Diabetic foot syndrome is one of the most often chronic and severe complications of diabetes mellitus. Method of local treat-ment of trophic ulcers and wounds in the setting of diabetic foot syndrome using autolymphocytes processed by leukinoferon was developed in the Institute of Clinical and Experimental Lymphology (Novossibirsk). Autolymphocytes were obtained by lymphocytapheresis on the blood cell separator «Haemonetics MCS+». The study of ultrastructural organization of identified lym-phocytes processed by leukinoferon did not reveal any disturbances of their structural integrity. Numerical density free polysomal ribosomes increased by 50%. Volumetric density of vesicular structures in cytoplasm of lymphocytes increased by 3 times. Number of excrescences and microvilli formed by plasmatic membrane increased by 2,4 times. Thus, technologies of autolymphocytes iden-tification do not result to damaged structure of cells. Processing of the lymphocytes by leukinoferon does not cause cellular damage but promotes development of structural changes.
For evaluation of potential staying duration of cells in peripheral blood on the character of cell kinetics, method of mathemati-cal modeling was used. Potential staying duration of cells in peripheral blood was numerically shown to deeply influence on anemia, but only for the small share of cells' death.
Prevalence of left ventricular hypertrophy in patients in dependence on blood pressure (BP) in professional groups of patients with high and low intense of job which is comparable with prevalence of risk factors ( smoking, hypertonic disease heredity, low physical activity, high body mass) and average levels of blood pressure is studied. The study included 853 males, working (engine-drivers and their assistants) with different levels of blood pressure (normal, high normal BP and hypertension). Controls were 330 males with lower intensity of labour. Higher (19%) prevalence of left ventricular hypertrophy was shown in engine -drivers with normal and high normal BP; prevalence in controls was 1,8 and 8,3% respectively. Patients with arterial hypertension revealed no such differences. Increased level of stress induced by high intense of labour is mediated by neurohumoral influences of stress hormones on myo-cardium. This can cause the development of left ventricular hypertrophy.
Peculiarities of phenotypic structure and factors of apoptosis induction (level of CD95-antigen cell expression, blood serum content of TNF-α ) of peripheral blood lymphocytes in patients with diffusive toxic goiter are discussed in thе article.
Aimed at non-specific reactivity of the organism, cytochemical activity and microbicidal properties of blood neutrophils and peripheral blood monocytes were studied in 102 patients with drug-sensitive pulmonary tuberculosis before the treatment, after in-tensive phase of therapy and after stopping anti-tuberculosis therapy. Increased hydrolytic enzymes' activity of leucocytes, levels of non- cationic proteins and decreased levels of cellular glycogen and induced NBT-test were observed without their dependence on drug-sensitivity of mycobacterial tuberculosis before and during the treatment.. At the same time, content of lipids in neutrophils and monocytes undergoes different changes.
Aimed at investigating influence of hypoxic hypoxia of different duration on haemostatic system state, the assessment of coagulogram in 152 Vistar rats 1 and 3 hours after single «lifting» to the height of 5500 m over sea level in pressure chamber and after daily 3 hours during 7 and 30 days. Activating influence of single hypoxic xypoxia was on hemostasis was revealed. While in-creasing duration of single impact , activation hemostatic system increased and anticoagulant and fibrinolytic properties of blood plasma decreased. According to the coagulogram data, prethrombotic changes developed in the form of coagulation activation, thrombocytes intake, decreased anti-thrombin III level and fibrinolytic activity on the 7-th day of hypoxia. On the 30-th day, de-creased hemostatic activity with increasing anticoagulant plasma blood reserve and restorating fibrinolytic activity.
Rat experiments revealed that daily inhalation of ultra-disperse powder (UDP) оf zirconate-titanate lead in a dose of 15 mg/m3 re-sult in increased contents of single element of UPD in kidneys and cause changes of morphological and functional status of the or-gan. According to accumulation degree in kidneys, the studied chemical elements are situated in the following sequence: Pb > Ti > Cr > Mn. The presented results are indicative of moderately expressed toxic effect of UDP of circonate-titanate lead on morphological and functional status of rat's kidney, been subjected to its inhalation influence.
LITERATURE REVIEW
Phenotypical and functional heterogeneity of immune regulating subpopulations of T-lymphocytes having suppressor activity is discussed in the article. Natural regulators and inducible regulating cells are characterized in more details.
The aim of the study was to investigate peculiarities of anxiety-depressive disorders (ADD) course in patients with bronchial asthma (BA) combined with erosive-ulcerative lesions (EUL) of gastro-duodenal area (GDA) and their interaction and to evaluate the treatment efficacy in this category of patients taking into consideration their psycho-pathological disorders. Combination of BA and EUL of GDA was established to enhance psychological tension degree and increase ADD which requires pharmacological cor-rection.
One of the most popular methods of myocardial revascularization is the surgery of coronary arteries. Our research results revealed that adequate myocardial blood flow is provided by hemo-perfusion from the left ventricle to coronary arter-ies.
FROM SCIENCE TO PRACTICE
Nourishment peculiarities of 120 patients aged from 41 years having excess body mass and obesity were investigated. Interre-lations with changed hormonal-immune status were assessed. Method of food taking rate developed in the Institute of Nourishment, RAMS, taking into account nourishing peculiarities of Russian population was used. Study results revealed, that patients had imbal-ance concerning main macro- and micronutrients: nourishment regimen was disturbed in 90% of cases, taking main part of energy value was shifted to the second half of a day, increased calorie content of nourishment, excess of monosaccharides in the setting of decreased physical activity were seen which plays the main role in the development and advancing obesity, increased level of leuco-cytes, of TNF-α, IFN-γ, humoral immunity link imbalance, increased serum concentration of leptin and insulin which is closely cor-relates with excess body mass and nourishment peculiarities.
Concentrations of some acute phase proteins (alpha-1-antitrypsin or AT; alpha-2-macroglobulin or MG; pregnancy associated alpha-2-glycoprotein or PAG) and lactoferrin (Lf) were studied in blood serum of 82 women with various types of uterine append-ages inflammation. The data obtained have been compared with conventional and commonly used laboratory indices of inflammato-ry response such as fibrinogen (Fbg) and leukocyte intoxication index. Our findings confirmed significant role of macroglobulin family proteins in the development of acute phase inflammation. Associated tendencies in changes of acute phase proteins concen-trations in various types of uterine appendages inflammation have been identified and recommended to be employed in clinical prac-tice for making differential diagnosis and for selection of optimal therapeutic approach.
ECONOMICS AND HEALTH CARE MANAGMENT
Aimed at decreasing number of conscripts having allergic dermatoses, therapeutic measures were developed directed to stop-ping main pathologic manifestations of the disease which consequently leads to improved quality of life. Diseases of the skin were decreased in the Tomsk region from 6,6 to 6,1% based upon results of call-up campaigns in 2003-2005 years. Questionnaire of 150 persons in the call-up age was performed aimed at identification of main social and clinical factors which lead to the develop-ment of allergic dermatoses. Schemes of clinical observation given in the article allowed us to perform adequate observation of pa-tients with socially important dermatoses, to stop in time acute manifestations of the disease and to decrease number of patients with the given pathology in the structure of coscripts who received delays or liberations from military service
Disturbed nourishing regimen was revealed in 54.9%, hypodynamia in 12.0%, distresses in 91.4% of workers. Prevalence of smoking was 44.9%, alcohol intake in 87.0%. Specific attitude to their health, medical aid, being informed about risk factors was revealed. Stable tendency to increased morbidity was revealed. Blood circulation diseases are on the 1-st place in the structure of to-tal morbidity.
Morbidity in chemical industry is due to labour conditions, absence of being interested in preserving their health. High mor-bidity is redoubled by unsatisfactory organizing and quality of medical -prevention service, bad material base. One should pay an attention to increasing results of medical-preventive service, labour conditions, health status, medical aid to the workers.
The aim of the study was performing clinical-economic analysis of the assessment methods of morphologic-functional status of left ventricular myocardium in ischemic cardiomyopathy patients. The assessment of expenses of morphologic study methods of before operation and intra-operation diagnosis of myocardial dysfunction irreversibility stage was done. Method of minimizing ex-penses was chosen to analyze these two approaches. Comparative analysis of morphologic methods showed that the most economi-cally profitable is before operation method. The economic assessment was performed for optimization of morphologic studies in the work of cardiovascular surgery department of the Institute of Cardiology, TSC RAMS (Tomsk). The necessity of myocardial dys-function state analysis is due to the selection of optimal approach to surgical treatment of ischemic cardiomyopathy patients or re-vealing contraindications to it.
SHORT MESSAGES
The 12-week open uncontrollable study of eprosartan organoprotective efficacy was performed in 15 patients with chronic glomerulonephritis. Study results showed high organoprotective efficacy of eprosartan, prescribed in a dose of 600 mg daily, which is expressed as anti-proteinuric, anti-hematuric actions, positive dynamics of intraglomerular hemodynamics parameters, decreased left ventricular hypertrophy and stiffness and also normalized vasomotoric functions of brachial artery endothelium.
