Abstract

Genomic imprinting is a form of non-Mendelian epigenetic inheritance that is defined by differential gene expression depending on its parental origin — maternal or paternal. It is known about 60 imprinted genes many of which effect significantly on the fetus growth and development. Methylation of DNA cytosine bases that defines the interaction of DNA and proteins identifying the modified bases and controls the gene expression through chromatin compacting-decompacting mechanism, is a main epigenetic genom modifier. Disturbances in monoallelic gene expression lead to the development of a special class of human hereditary diseases — genomic imprinting diseases.

Abstract

The antimutagenic activity of 41 Siberian herbs has been studied with microcores analysis method (calculation of micronucleous erythrocyte number in peripheral blood of mice). The most active species have been composed in herb preparations and the preparation with the most activity has been revealed with microcores analysis method. The microcore number decrease has been observed up to 1,80‰. Antimutagenic herb preparation activity is defined by the content of vitamins, pigments, coumarins, polyphenol complex, flavonoids, terpenic saponins, lactones and other compounds.

Abstract

The role of transforming growth factor β (TGF-β) in atopic dermatitis pathogenesis is discussed basing on the analysis of existing data of cellular and molecular mechanisms of allergic inflammation. Up-to date data of the main T-helper (T-h) lymphocyte subpopulations including Tx1, Tx2, Tx3 has been presented. Functions of regulatory T-cell populations and produced cytokines have been described. The main attention has been accented on the TGF-β structure and biological activity as a main Tx3 cytokine. The current information of TGF-β influence on different cell populations and its biological activity realization mechanism is thoroughly discussed. Information relating to the mechanism of cytokine regulation during atopic dermatitis has been summarized. A deep analysis of possible participation of TGF-β in disbalance formation on Tx1 and Tx2 levels, in disturbances of histological derma structure and allergic inflammation timing has been made.

Abstract

Historical and modern data of the need of the creation of social work faculty in Siberian State Medical University is discussed in the present paper. The need of university education renovation in the sphere of mental health protection is a ground of the social specialist training. It is suggested a development of biopsychosocial education model that requires the training of social workers in the sphere of clinical psychology, psychotherapeutic supports and psychiatry. The analysis of the current state of social service efficiency in Russia reveals the need of the training of new generation specialists for the development of the social net, social assistance, support and rehabilitation of persons having temporary or chronic mental and behavioural disorders or in crisis living situations.

Abstract

192 patients with chronic opisthorchosis pancreatitis (117 women (60,9%) and 75 men (39,1%) being under observation in Tomsk regional hepatologic center from 1970 till 2000) have been included in the protocol of retrospective cohort study. 36 patients (18,8%) have had a chronic pseudotumor-like pancreatitis. 133 patients (69,9%) have been diagnosed a chronic painful pancreatitis, 23 patients (12,0%) have been diagnosed a pancreatic cyst. Three basic forms of the chronic opisthorchosis pancreatitis have been suggested: a chronic painful pancreatitis, a chronic pseudotumor-like pancreatitis and a pancreatic cyst.

Abstract

138 patients with lumbar osteochondrosis (LOC) in age of 32—73 years with apparent pain syndrome and neurologic manifestations have been examined, these patients have received 316 autoserotherapy (AST) procedures. It has been revealed that in LOC patients a disturbance of coagulation blood properties and micricirculation as well as immunoglobulin M and G concentration increase, circulating immune complexes (CIC) increase, increase of kinin activity by more than 2 times had taken place. Course of AST procedures permits to decrease the blood coagulability and kallikrein activity almost up to the norm, to improve the microcirculation (by capillaroscopy data), to eliminate the disimmunoglobulinemia that is accompanied by the decrease of disease symptom evidence. Inclusion of AST procedures in LOC treatment complex decreases in short time the intensity of lumbar pains, decreases the hospitalization duration by (9,4 ± 1,1) days, increases the remission duration as compared to the control patient group that received the traditional treatment only.

Abstract

In this paper an epidemiological situation regarding tick-borne encephalitis at children in Tomsk region is analyzed. It is noted that Tomsk region is an endemic center of tick-borne encephalitis with cyclic variations by subareas. The total number of children with tick-borne encephalitis in the south and middle subareas is greater then in the northern subarea. The number of diseased children depended on the number of persons went to seroprophylaxis stations after the tick suction, on the tick number and duration of epidemiological season. Vaccination, accessible and free specific seroprophylaxis of the population promoted the step-by-step increase of feverish and meningeal form part and the decrease of focal tick-borne encephalitis form part.

Abstract

In this article a significance of a negligence of health care institutions when bringing them to civil liability has been investigated. A common rule of assignment of responsibility only when guilty has been examined as well as an exclusion of responsibility in case of increased (guiltless) liability. Examples have been given and a legal treatment of situations when a mutual contributory negligence took place (executor’s negligence as well as recipient’s (patient’s) negligence) has been presented. The significance of presumption of innocence in civil law has been revealed.

Abstract

Rural treatment-and-prophylactic institutions (TPI) and mainly central regional hospitals (CRH) – the key rural health protection units – insufficiently control and improve the medical service system quality due to a number of reasons. In rural CRH a number of incomplete examination cases and, consequently, a number of wrong diagnosis cases is higher significantly. Incomplete treatment becomes evident in rural areas more frequently as well. As a result the treatment quality in rural CRH is lower significantly than in Tomsk TPI. It is necessary to draw a special attention to the problem of rural medical service quality increase and to liven up the control of privided medical service quality in regional centers.

Abstract

Practice of treatment standard application experiences the certain difficulties. On the one hand, the most treatment standards lay on insufficient economical base, on the other hand, the limitation of health protection in bankroll leads to the insufficient treatment efficiency.

An analysis of diagnostic study costs in Tomsk special treatment-and-prophylactic institutions has been made on the example of inflammatory pelvis minor organ diseases.

The results of the investigation have shown that the character of diagnostic study of patients with inflammatory pelvis minor organ diseases differed significantly from the treatment standard. At the same time this fact defines not only the necessity of expense increase for diagnostic study but, first of all, the necessity of changes of diagnostic study structure that has been proved by modern pharmaeconomical methods.

Abstract

Use of endoscopic ultrasonography in diagnostics of rectal low-ampullar malignant neoplasms has permitted to the authors to define the list of intersphincteric resection indications consisted in total mesorectumectomy and rectal and interior sphincter resection. The method of spheroidal reservoir formation with artificial unstriped muscular sphincter-cuff has been developed for the prophylaxis of anal incontinence and multimoment emptying after intersphincter resection with the colon bringing down. 14 patients have been operated with survey time up to 3,5 years. According to the data of clinical investigations the spheroidal reservoir form with small initial size and with the ability to extensibility as well as autonomous obturator function of striped muscular sphincter have become the basic conditions for the restoration of continence act and stool frequency in far period after intersphincter resection.

Abstract

The aim of this investigation was to define the concentrations of serum proteinase — plasmin — and some of proteinase inhibitors, such as α2-macroglobulin (MG) and α1-antitrypsin (AT), in order to study their role in cerebral infarction (CI) and discircular encephalopathy (DE) pathogenesis. We have investigated 30 patients with cerebral infarction, 30 patients with discircular encephalopathy and 60 serum samples of the healthy people as a control. Plasmin (Pl), AT and MG levels have been defined with rocket immunoelectrophoresis method. We have found a significant Pl decrease in both group patients as compared to the control. The decrease has been (95,83 ± 4,3) mkg/ml in CI patients’ serum, (83,49 ± 3,79) mkg/ml in DE patients and (112,98 ± 2,66) mkg/ml in the control group. AT concentration has been increased in DE patients’ serum [(2,44 ± 0,15) g/l], MG concentration has decreased significantly [(1,08 ± 0/09) g/l] as compared to the control. On the contrary, MG serum concentration (1,72 ± 0/08 g/l) in CI patient group has not differed from the control [(1,64 ± 0,04) g/l] but it has been accompanied by AT level increase [(2,50 ± 0,16) g/l at patients and (1,78 ± 0,04) g/l at control]. We think that these differences can be used during differential diagnostics of the investigated diseases.

Abstract

The life of the outstanding person, famous Russian and Soviet surgeon Alexander Alexandrovich Opokin is a striking example of whole-hearted service to the Motherland and medicine. Being a pioneer of thoracic and oncologic surgery in Siberia, a brilliant field-military surgeon and a teacher of a whole pleiad of disciples, he has contributed a lot into the development of domestic surgery. The 125-th birthday anniversary A.A. Opokin is a significant reason to recall his name and actions, to bow down before him and to compare the own life guiding line.
