Abstract

In the lecture the modern views on diagnistics and tactic choice in the treatment of complicated and uncomplicated gastroduodenal ulcers have been presented and long-term results of organ-saving and organ-modeling surgery experience during complicated stomach and duodenal ulcers have been analyzed. Achievements in life quality saving of operated patients have been cited. Novel classifications of gastroduodenal anastomosis functional activity and anastomositis degrees have been proposed.

Abstract

The biography of an outstanding scientist, a founder of the chair of obstetrics and gynecology, an ordinary professor of the Emperor’s Tomsk University (now SibSMU) I.N.Grammatikaty, one of the creators of Russian obstetric and gynecologic literature, has been presented in this article. Here are presented materials reflecting the formation and development of the chair of obstetrics and gynecology and the main scientific achievements of the chair for the last 10 years.

Abstract

The aim of the work is to study the efficiency of different antibacterial medication combinations for the treatment of patients with acute Lyme mite borreliosis. 156 patients aged 20—76 have been on regular medical check-up. Different penicillin and tetracycline antibiotics have been applied as well as cephalosporins of 1 and 3 generation. The treatment duration has been 7—14 days. The duration of general intoxication syndrome, terms of clinical symptom dissapearance, antibody titer dynamics in indirect immunofluorescence reaction with borreliosis antigen, presence of side actions and long-term results of the treatment have been taken into account in the evaluation of different antibiotic combination efficiency. 1 year after discharge from the hospital 69,2% of observed persons considered themselves healthy, 15,4% obtained the diagnosis of chronic clinical LMB with predominant joint or nervous system damages. Thus, analysis of the investigation result shows that currently used etiotropic therapy regimens are not sufficiently effective. They allow recovery of 69,2% of patients with acute Lyme mite borreliosis only.

Abstract

The results of the study of gastroesophageal reflux disease (GERD) clinico-endoscopic peculiarities in 318 patients with bronchial asthma (BA) of different severity degree, combined with chronic opisthorchosis or without it, have been cited in the present article. It has been revealed an often incidence of GERD in BA patients and GERD frequency increased as the disease severer. At that the GERD clinical manifestation have been observed more frequently than the reflux-esophagitis had been discovered by endoscopic ways. GERD structure during BA has been presented. More frequent GERD with BA combined with chronic opisthorchosis has been discovered than in patients without associated parasitic invasion.

Abstract

On the base of experimental and clinical investigations there has been found that continent reservoir ileorectostomies after colectomy under the rectal contents effect had been exposed to degenerative and dysplastic mucous changes. These damages prohibit from stool formation and increase the risk of reservoir malignization. A method of continent reservoir ileorectostomy formation with restraining and areflux mechanism has been developed for pathological transformation of ileal structure. Results of morphological and instrumental investigation methods made on 33 dogs and 6 patients after colectomy show that the restraining and areflux mechanism permits to avoid displastic and degenerative changes in ileal reservoir mucous coat and to recover the stool formation due to motor-evacuation function regulation and keeping the constant physical, chemical and bacterial reservoir and rectum properties after colectomy.

Abstract

During experimental Reye’s syndrome caused in rats by 4-pentenoic acid, histoarchitectonics disturbances, circulatory disturbances, microvesicular steatosis, necrosis in central lobule part have been developed, number of 2-nucleus cells has been decreased, activity of mitochondrial, lysosomal and cytoplasmic hepatocyte enzymes has been disturbed. Hepatoprotectors of polyphenol nature — lochein, maksar and legalon — used for experimental Reye’s syndrome therapy, have regenerated a normal liver histologic structure, decreased the microvesicular steatosis and the quantity of necrotizing parenchyma cells, stimulated the regeneration and increased the activity of intracellular hepatocyte enzymes. Lochein and maksar possess the most therapeutic activity.

Abstract

Lactobacillus are an important component of the human and animal resident microflora. Lactobacillus possess a pronounced antagonist activity with respect to pathogens and conditionally pathogens as well as to other kinds and even to lactobacillus strains. Lactobacillus have immunomodulatory and antineoplastic effects, decrease cholesterol content and synthesize vitamins and other bioactive substances. Along with the positive effect on the human health lactobacillus may be a reason of diseases of people with secondary immunodeficiency. L. casei sp. rhamnosus, L. plantarum, L. brevis, L. lactis, L. fermentum, L. acidophilus and L. salivarius have been registered as causative agents. Hyperlactacidemia syndrome of children during bowels colonization by L. fermentum and L. buchneri has been described. Some L. buchneri strains are able to synthesize histamine that may be a reason of food toxicosis. Lactobacillus strains used as probiotics for correction of resident microflora imbalance as well as in production of functional foods, should not possess the properties, negatively affecting a human organism.

Abstract

1170 patients with chronic opisthorchosis have been operated by surgeons of Tomsk hepatology center for 35 years, 4756 patients have been treated. In 730 operated patients different strictures of biliary tracts have been revealed, strictures of cystic duct have been observed in 372 patients (51%), strictures of choledoch distal section and major duodenal papilla (MDP) of expanded character — in 124 patients (17%), stricture combination — in 205 patients (28%) and sclerosing cholangitis — in 29 (4%).
The treatment of opisthorchosis strictures of biliary tracts is surgical. In case of any its localization a cholecystectomy is necessary, in case of strictures of choledoch distal section and MDP a supraduodenal choledochoduodenostomy is necessary. In early postsurgical period it is prescribed an anthelmintic therapy with iodinol through external drainage of bile ducts. Good farther results have been marked at 79,4% of patients.

Abstract

In the present investigation it has been foreseen to develop pathogenetically founded methods of the chronic pain syndrome relief in patients with disseminated prostate cancer. 47 men with the prostate cancer of III—IV stage have been examined and treated. It has been stated that either a long-term epidural sensory block created with hydrochloride morphine or anekaine, or an intrathecal hormonotherapy with synesthrol and hydrocortisone had significantly decreased the pain syndrome and the level of prostatespecific antigen in blood serum, had significantly improved the urodynamics increasing an average and maximum rate of urine flow and urine volume, reducing the time of urination, and had improved the patients’ life quality by 17—30%.

Abstract

The aim of investigation is to study blood rheological properties in patients with chronic generalised periodontitis combined with gastrointestinal tract disturbances. 205 patients have been examined including those with erosive ulcerous involvement of gastroduodenal area — 51, with stomach ulcer and duodenal ulcer — 62, with chronic colitis — 39, with nonspecific ulcer colitis — 53. Blood viscosity properties, indices of erythrocyte aggregation and deformability, rate of oxygen delivery to tissues have been evaluated by rotary viscometer АBR-2 (Russia).
It has been revealed that blood rheological properties in patients with chronic generalised periodontitis combined with gastrointestinal tract disturbances had been disturbed, that was accompanied with the blood viscosity rise, erythrocyte aggregation increase and with the decrease of deformability and rate of oxygen delivery to tissues. The degree of violations in blood rheology indices was mostly expressed under chronic and plural erosions of gastroduodenal area, duodenal ulcer and severe course of chronic nonspecific ulcer colitis.

Abstract

A cancer incidence in general and in its separate nosologic forms (stomach cancer, liver cancer, cancer of lung, skin cancer, breast cancer, cervical cancer, prostate cancer, thyroid gland cancer and hemoblastosis cancer) has been studied in the population of Tomsk and Tomsk region within 1990—2001 taking into account ages and sexes. The dynamics of the cancer incidence in general and in separate forms of malignant neoplasms has been cited and the prognosis of this process up to 2007 has been done. An analysis of the main risk factors has been made.

Abstract

In the article it has been shown the efficiency of children detection with bronchopulmonary pathology and risk of its forming using the «EDIFAR» computer system. Screening with this computer technology allows in short terms a pictorial view of children’s health on the primary health service territory. This investigation has been made in pediatric polyclinic ‹ 4 in Blagoveshchensk. The results show the actual prevalence of respiratory diseases at children aged 0—7 years and are an evidence of a significant exceeding of disease indices as compared to the official statistic data. Information get with the computer system is necessary not only for address aid but is useful in conditions of insurance medicine and market principles of funds spending for health protection as well.

Abstract

The aim of the investigation is a determination of diagnostic value of whole blood low-frequency piezoelectric hemocoagulography method (LPH) and its suitability for monitoring the homeostasis functional state during the burn shock. 40 patients with the severe thermal injury and different consequences of burn disease have been examined in development dynamics and intensive burn shock therapy with LPH method and general biochemical tests that reflected the state of vascular platelet and coagulation homeostasis and fibrinolysis components. It has been found that severe burn shock development has been accompanied by significant pathological changes in all hemostasis sections, the rate and severity of which has been connected closely with the outcome of the burn disease. The LPH method is the easiest and most informative one for monitoring the functional states of all hemostasis sections during the dynamic critical state, such as the burn shock.

Abstract

In the article there have been discussed the consequences of Listeria internalization and phagocytosis depending on hepatocyte and macrophage receptors type (Met, gC1q-R, C3b-R, Fc-R). It has been supposed that internalization and phagocytosis variant depended on the stage (inductive stage, IgM synthesis stage, IgG synthesis stage) of immunogenesis developing during an infectious process. It has been stated that internalization and phagocytosis mechanism was alternative and defined the destiny of a causative agent in hepatocyte and macrophage as well as the possibility of granulomas formation and of their reverse development.
