Abstract

The article reports about main directions of scientific researches on the chair of department pediatrics with course of children diseases of medical department of Siberian State Medical University during 70 years period. The researches were conducted in three main directions that are acute nutrition disorder and indigestion among children of young age, diseases of gall-bladder and biliary tracts among children, and also opisthorchiasis – one of the most widespread under conditions of West Siberian helminthic invasion. At the present moment special attention in scientific researches of the chair is paid to study of atopy activities on opisthorchiasis phone.

Abstract

It was determined the effectiveness of scheme of treatment of children with chronic inflammatory diseases of upper part of digestive tract, associated with Helicobacter pylori. The scheme included: De-nol, Rabeprazole, Amoxicillin, Macmiror (DRAMa). In the research took part 47 children in the age 9,0 ± 0,2. It was acquired 100% eradication level of Helicobacter pylori.

Abstract

The regional clinical features, parameters of local protection at 143 children till 3 years, patients with rotaviral infection are investigated. The inclusion is proved in therapy antirotaviral antibodis of oral introduction.

Abstract

The aim of the research is to explore prevalence of celiac disease in groups and clinical- immunologic and genealogical characteristic of celiac diseases among children of Siberian region. It was examined 469 persons from celiac disease risk groups. There were conducted biopsy of mucous coat of thin bowels, determination of level of antiagliadine antisubstances, serum immunoglobulins, genetic methods of research.  Prevalence of celiac diseases among children of Tomskcity on January, 1, 2005 comprised 32,66 on 100 000 children. There was registered high level of antiagliadine antisubstances among celiac disease risk groups (autoimmunologic thyroadenitis, diabetes mellitus of the 1st type, relatives of the first relation degree). In proband families with celiac disease there was discovered high load in chronic pathology of gastrointestinal tract.  There was determined investment of polymorphism VNTR gen among children and adolescents.

Abstract

The aim of research is to estimate clinicopathologic aspects of hepatobiliary system abnormalities at leading endocrinous diseases among children and adolescents.

There were examined 140 children and adolescents with thyroid gland pathology and 125 children with diabetes mellitus of 1 type. Comparison group consists of 30 children. There was estimated hepatobiliary system condition using fractional duodenal intubation, bile biochemistry, US of liver and gall-bladder, dynamic radionuclide hepatobiliary imaging. It was determined that changes from side of hepatobiliary system depend mostly from functional characteristic of thyroid gland than from duration of disease at pathology of thyroid gland. Changes in biochemical composition of bile at thyreopathy were conditioned by abnormality of biliary excretion motor activity. Clinic-functional changes from hepatobiliary system occurred in 50% of cases and fatty hepatosis was mostly frequent diagnosed at SD of the 1st type. There were suggested complex of diagnostic examination in order to define pathology of hepatobiliary system at endocrinopathy.

Abstract

It was studied 58 patients with medium size ulcer and 182 with big size ulcers in order to compare activity of stomach ulcer and duodenum disease of patients with medium and big ulcerous defects and discover its peculiarities depending on sizes of ulcers. It was discovered that big size ulcers occur more frequently among the age between 40–49 (32.42%) and more often in stomach than in duodenum. High secretory function of stomach is discovered among most of patients.

Abstract

The prevalence of H.pylori virulence genes (iceA, cagA, babA2, vacA) as well as the distribution of the IL-1 genes was studied in patients with H.pylori-associated gastric and duodenal ulcer disease. VacA s1/m2 iceA1 cagA+ и vacA s1/m2 iceA1 cagA+ babA2 genotypes of H. pylori were shown to be prevalent in the clinic isolates. Four patients were found to be infected with Ureaplasma urealyticum. The combination of the IL‑1B ‑511*Т / IL‑1B +3954*C / IL-1RN*2 alleles was shown to be prevalent among the patients with gastric and duodenal ulcer disease. The correlation between the H. pylori genotypes, the IL-1 genes and the peculiarities of ulcer disease was not found.

Abstract

In order to study the effectiveness of ozone therapy method there was conducted treatment of children with different forms of allergic dermatitis for 28 days. Ozone therapy as a treatment method can be used in clinical practice in combination with other physical factors and medicinal preparations. Frequency of conduction of repeated courses depends on the character of the disease and may be 1-3 cycles a year.

Abstract

Application of complex therapy with inclusion of vegetotropic drugs in treatment of patients with bowels irritation syndrome favors normalization of vegetative indexes among most of patients. At the same time effectiveness of the conducted therapy depends from initial indexes of vegetative activity.

Abstract

In the world frequency of patients affected by celiac disease comprises 1 per 184 persons.  The disease is provoked by using gluten contaning products for food. Dietotherapy is the only and effective way to treat children affected by celiac disease. Wheat, rye, barley and oats are excluded from nutrition of children affected by celiac disease. Among cereal products it is allowed to eat corn, rice, buckwheat, millet. For nutrition of children with celiac disease it is recommended to use certified products.

In the review it is observed the importance of determination of serum antibodies of different classes (IgA, IgG) to gliadine, reticulin, endomysium, tissue transglutaminase for diagnostics and screening under celiac disease, advantages and disadvantages of methods, peculiarities of immunologic reactions under celiac disease among children of different age, dynamics of titres of antibodies on the background of observance and non observance of gluten-free diet, place of immunologic methods in complex diagnostic of celiac disease and for control for diet observance.

Abstract

Hepatolenticular degeneration or Wilson's disease is the hereditary disease, caused by derangement of copper metabolism and its precipitation in brain and internal. Stages of disease study and development of its nosologic independence are emphasized. Molecular genetic mechanisms of hepatolenticular degeneration, its pathogenesis and clinical classifications are stated. Principal laboratory indices and pathogenetic therapy of Wilson’s disease is represented.

Development of autoimmune and oncological deiseases under celiac disease determines social sinificance of opportune diagnostics of celiac disease.  It was given a very rear case of combined activity of celiac disease with Crohn's disease and somatotrophic deficiency of the 16 years old child.

Abstract

The aim of the research was to study the influence of chronic gastroduodenal pathology on life quality indexes of 97 children with chronic duodenitis and also there was studied attitude of parents (cross-form research) to health condition of their children. There were used standard gastroenterological methods of diagnostics and treatment. Estimation of life quality was conducted in dynamics by SF-36 questionnaire.

The research detected that gastroduodenal pathology influence not only functions of gastrointestinal tract but also significantly reduces indexes of general health, vital capacity, limits social functioning of the individual, after course of eradication therapy patients are in need of additional rehabilitation, duration of therapy and observance at exacerbation of chronic gastroduodenitis must be not less than 3 months from the beginning of treatment, even if there are no complaints and physical changes.

Abstract

There were studied possibilities of ezomeprazol in therapy of functional dispersion (FD) among 11 children of 8–12 years old according to data of daily pH-monitoring. It was find out that the drug provides fast and long-term suppression of acid-forming function of stomach and relieve epigastric pain effectively from the first day. Among some children with FD when application of the drug there can be observed asymptomatic episodes of night acid outburst and also resistance to the drug that is overcome by means of increase of dose to overage one.

Abstract

It was studied development of complications among 240 patients with stomach ulcer and duodenum disease with different sizes of ulcerous defects. It was discovered that among patients with big size ulcers complications occur more frequently and depend on duration of disease; bleeding is among the most frequently occurred (56.66%).
