Abstract

Proctectomy with total mesorectal excision, resection of the internal sphincter, deep portions of the external sphincter and reductionwas performed on nine patients aged from 38 to 74 years with adenocarcinoma of low rectal cancer (T2N0M(n = 4), T2N1M(n = 1), T3N0M(n = 3), T4N2M(n = 1)), located in 8–25 mm from an anus. Simultaneously, atypical resections of the liver were performed on two of the patients. In the perineal section of the reduced intestine a sphincter was created from a smooth muscle cuff, a levator from not free serous, and a muscular flap, fixed to a forward wall of the small basin, and a reservoir of cross stitched submucosa site formed after the formation of a levator. 
5 of the patients (T2N0-1M0)had no disease advance 3–9 years. They were able to hold firm feces, but were less able to hold liquid feces, and sometimes didn’t hold intestinal gas or differentiate between feces and gas, but were capable of ignoring an urge to defecate and to detain a defecation (level of a fecal continence on a scale of "Wexner score" (11 ± 1) of points).

Abstract

Objective: research gender features of relationship of hormonal activity of adipose tissue and proinflammatory status with essential hypertension (EH) in combination with metabolic syndrome (MS).

Material and methods. Were examined 46 patients with essential hypertension stage (BP < 180/110 mm Hg.) in conjunction with the metabolic syndrome. Along with a complete clinical, laboratory and instrumental examination adopted in specialized cardiological clinic, were defined concentrations in serum leptin, adiponectin, resistin and visfatin and the level of markers of systemic inflammation (C-reactive protein, neopterin and fibrinogen), was assessed the level of spontaneous production of cytokines and active oxygen species and the expression level of CD4, CD8 and CD36 – blood mononuclear leukocytes markers.

Results and conclusions. It was established that the hormonal status of adipose tissue have women with EH combined with MS are characterized by higher levels of leptin and adiponectin in blood serum than in men.

It was established not only a significant role adipokin imbalance in mechanisms of MS and systemic inflammation in these patients category, but also were studied gender characteristics. While for men in the development of the above pathological manifestations hipoadiponektinemia has the most meaning, and for women – hyperleptinemia.

Abstract
The practical aim of the research is to develop a methodology for assessing the functional state of the musculoskeletal system in the case of complex human pathologies and associated injuries with the objective distinction of interdependent anatomical and functional disorders in the osteoarticular system, nervous and muscular subsystems, identification pathology severity in the separate subsystems and the degree of influence for each of them on the movement organization as a whole. In patients with cerebral palsy the neurological movement disturbance is a priority component, differentiation of which may by difficult due to lack of tools to evaluate objectively and display the features of functioning nervous system in normal and pathological conditions. The authors propose a set of interval indicators of stabilometric study and method to use it providing the more qualitative consideration individual characteristics and the formation of clinically interpretable representation of a visual results image as compared with the standard vector and frequency indicators.

Abstract

Objective. Investigation of hypolipidemic effect of sesquiterpene γ-lactone ahillin in hepatoma tissue culture (HTC) cells.

Material and methods. In this study we’ve evaluated the effect of γ-lactone sesquiterpene aсhillin and gemfibrozil (comparator drug) on the lipid content in the hepatoma tissue culture (HTC) cell which were incubated with a fat emulsion lipofundin by fluorescent method with vital dye Nile Redand staining the cells with the dye Oil Red O. The cell viability was investigated using the MTT-test and staining with Trypan blue.

Results. Cultivation cells HTC with aсhillin and gemfibrozilat concentrations ranging from 0.5 to1.5 mM and from0.25 mM to0.5 mM, respectively, resulted in dose-dependent decrease of the fluorescence’s intensity Nile Red. It reflects a decrease in lipid content in the cells. At these concentrations the drugs didn’t have cytotoxic effect and the cell viability didn’t change compared to the control culture.

An experimental hyperlipidemia in the hepatoma culture cells was induced by adding to the incubation medium a fat emulsion lipofundin at a final concentration 0.05%. The intensity of fluorescence Nile Red in the cells was increased 4 fold (p < 0.05). This result suggests the significant accumulation of lipids in the cell’s cytosol and confirmed by microscopy after staining neutral lipids with the dye Oil Red O. Under these conditions aсhillin and gemfibrozil reduced lipid content in cells and hadthe effect at concentrations of0.5 mM and0.25 mM respectively.

Conclusion. In the lipofundin-mediated model of hyperlipidemia the sesquiterpene lactone aсhillin prevents the lipid accumulation in cells. It confirms by decrease of fluorescence Nile Red and reduction lipid drops which were stained with Oil Red O in cytosol. To establish the molecular targets of aсhillin’saction on lipid metabolism in cell culture HTC we need to investigate a gene expression of key enzymes of lipid metabolism.

Abstract
This article describes an approach to the assessment of motion activity of man in after-stroke period, allowing the doctor to get new information to give a more informed recommendations on rehabilitation treatment than in traditional approaches. Consider description of the hardware-software complex for determination and analysis of motion activity after-stroke patient for the video stream. The article provides a description of the complex, its algorithmic filling and the results of the work on the example of processing of the actual data. The algorithms and technology to significantly accelerate the gait analysis and improve the quality of diagnostics post-stroke patients.

Abstract

Eosinophils are polyfunctional leukocytes detected in excess in blood and in the focus of granulomatous inflammation in pulmonary TB.

The research objective was to evaluate the adhesive properties as well as cytokine-secretory and antibacterial activity of blood eosinophils in pulmonary TB.

The research has been conducted on eosinophils isolated from peripheral blood of 43 patients with freshly identified progressive destructive TB with and without eosinophilia. Using flow cytometry and ELISA, expression of CD9 and CD18 adhesion molecules on blood eosinophils has been studied along with the phagocyte and cytokine-secretory functions and activity of eosinophil granulocyte peroxidase.

As a result of the research it has been established that in TB patients with eosinophilia the number of CD18-expressing eosinophils rises, whereas the amount of CD9+ remains within norm. Activation of the phagocyte function of blood eosinophil granulocytes is associated with the decrease in eosinophil peroxidase activity, while the increase in IL-5 and TNFα secretory reactivity is connected with oppositely directed changes in  IL-2 basal secretion by eosinophils in vitro (a fall in infiltrative TB and a rise in disseminated TB).

Abstract

The aim of the study was to examine age-related changes in the ultrastructure of synapses in the CA1 region of the hippocampus of Wistar rats and premature aging OXYS rats.

To assess changes of the sinaptoarhitectonics photographed on 15 randomly chosen fields of view of the pyramidal layer of the CA1 region of the hippocampus with five slices at a standard magnification of 10000. It was determined the amount of mineral contacts (size of the field of view is 50 μm2) and calculated numerical density of synapses per 100 μm2. The number of contacts with asymmetric and symmetric organization sistemasubsidiaries units, perforated, hypertrophic contacts are counted.

The study showed that there are significant differences in the ultrastructural organization of the synapses between the lines, already 4 months old. The numerical density of snaps in this age group OXYS rats was significantly higher than that of Wistar. Ultrastructure of synapses in the CA1 region of the hippocampus of rats at the age of 18 months more bulupodadan pathological essentiaompetencies processes and membrane prematurely senescent rats in contrast to the control line. The numerical indicator of protect contents 18-month-old rats OXYS was significantly lower than in Wistar.

Identified interstrain differences in the degree of changes of synapses can testify in favor of a different sensitivity to oxidative stress in rats investigated lines.

Abstract

The article summarizes the results of studies on the composition of microbial communities in stool samples of patients with chronic obstructive pulmonary disease (COPD) compared with healthy volunteers using genome-metagenomic sequencing. It is shown that the microbial community of the intestine in COPD patients is characterized as diverse taxonomic composition of metagenomes that the healthy volunteers microbiota. In this case, the normal composition of intestinal microbiota in patients with COPD is exposed to the qualitative and quantitative modifications. In contrast to healthy volunteers, the intestinal microbiota in patients with COPD is characterized by the presence of representatives of the Proteobacteria, as CitrobacterEnterobacterEggerthella,ProteusSalmonellaAnaerococcusClostridium difficilePseudomonas, as well as higher insemination fungus genusCandida (Candida dubliniensis and Candida albicans).

Abstract

4-methyl-2,6-diisobornilphenol (dibornol) – a promising drug with a hemorheological activity. Glycophorin A is one of erythrocyte membrane proteins involved in the aggregation and possibly mediating rheological effects of dibornol.

Objective: to conduct a modeling of the interaction dibornol and glycophorin A by AutoDock and HexServer programs.

Material and methods. We used three-dimensional models of molecules dibornol and glycophorin A. Information on the three-dimensional model of glycophorin A was received from the database RCSB Protein Data Bank – 1AFO. Modeling the three-dimensional model of a dibornol (4-methyl-2,6- diisobornilfenol) was modeling by PRODRG Server.

Results. This paper presents the results of computer modeling of interaction dibornol and glycophorin A by HexServer and AutoDock programs. We used the electrostatic properties of the molecule glycophorin A, site of interaction is position chain A VAL83, chain B – ALA82, GLY83, GLY86, THR87. The energy of binding was –6.73 kcal/mol by AutoDock program, HexServer – –2.89 kcal/mol. The charge of the molecular complex dibornol-glycophorin A decreased to –4.126 (the charge of the native molecule glycophorin A – –4.003).

Conclusion. Integrated use of the program AutoDock and HexServer helps significantly reduce the time and computational resources in the modeling. The study identified the amino acids that may play a key role in the interaction with dibornol glycophorin A. This study has given us reason to believe that as a result of such interaction dibornol may prevent adhesion of red blood cells.

Abstract

The purpose of the research was to estimate the prevalence of Streptococcus pneumoniae nasopharyngeal carriage in 250 children aged under 5 years and 215 school-aged children in Krasnoyarsk Region, and also investigate molecular epidemiological features of the strains circulating inKrasnoyarsk city and in Krasnoyarsk Region. This study is carried out for the first time in the region and provides a foundation for the choice of optimal vaccination strategy of pneumococcal disease.

S. pneumoniae identification was carried out using culture method as well as PCR (revealing of cpsA gene), serotypes were determined with PCR (according to CDC protocol).

The prevalence of S. pneumoniae carriage in under-five children was 51,6% with predominated pneumococcal serotypes 23F, 19F and 6В; most of these strains (up to 90%) belonged to those included in pneumococcal vaccines which are permitted for use in Russian Federation; the greatest conformity was observed in 13-valent conjugated vaccine. In school-aged children the S. pneumoniae prevalence was 12,1% with predominated pneumococcal serotypes 6В, 9VA and 15АF, most of these strains (up to 60%) belonged to those included in approved pneumococcal vaccines.

Considering high level of S. pneumoniae carriage in children under 5 years in organized groups and unorganized children from families with two or more children it is necessary to carry out vaccine prevention of pneumococcal diseases with of 13-valent pneumococcal conjugated vaccine application.

Abstract

The research objective is to determine the role of the gluthatione system components in realization of the receptor pathway of Jukart tumor cell apoptosis.

Apoptosis realization using FITC-labeled annexin V and propidium iodide as well as the amount of TNF R1- and Fas-presenting cells has been evaluated by flow cytofluorometry; activity caspase-3 registered a spektroflyuorimetrichesky method. The concentration of reduced and oxidated gluthatione has been determined by spectrophotometry.

The material for the research was intact Jukart tumor cells and the ones incubated in the presence of a selective inhibitor of the key gluthatione synthesis enzyme – buthionine-sulfoximine.

The research has shown that the gluthatione system plays an important regulatory role in activation of the receptor pathway of Jukart tumor cell apoptosis.The gluthatione system components are targets for activation of programmed cell death in tumor growth.

Abstract

Aim of this study was to investigate of NAD(P)-dependent dehydrogenases activity in lymphocytes at different outcomes widespread purulent peritonitis(WPP). Enzyme activity in blood lymphocytes was determined by bioluminescent method. Found that patients with a poor outcome of WPP in blood lymphocytes made more pronounced reaction of lipid anabolism and catabolism of xenobiotics. Regardless of the outcome of the disease in blood lymphocytes of WPP patients reduced levels of enzymes activity that determine the intensity of anaerobic and aerobic respiration and the level of plastic processes, but the activation reactions of lipid catabolism and transport products through glycerol-3-phosphate dehydrogenase on reactions of glycolysis. Based on the informativeness of the NAD(P)-dependent dehydrogenases activity in the classification neural network model for the disease outcome, metabolic coefficient is proposed, which has prognostic significance.

Abstract

Was investigated 67 children (36 boys and 31 girls) in age from 6 to 18 years old, with type 1 diabetes patients who were treated at the Children's Hospital Endocrinology number 1 inTomsk, average age of the boys at the time of the survey amounted to (13.0 ± 0.4) years, girls – (13.5 ± 0.4) years. All children diagnosed with type 1 diabetes were under subcompensation. Clinical evaluation of patients included a medical history, physical examination, evaluation of physical development. A study of lipid profile (high-density lipoprotein (HDL) cholesterol, low-density lipoprotein (LDL), cholesterol (TC), triglycerides (TG), atherogenic index). Screening for diabetic retinopathy was performed by examining the fundus by direct ophthalmoscopy after pupil dilation. Diagnosis of diabetic nephropathy, retinopathy performed according to the classification. Patients were divided into groups depending on the presence of complications, and the level of compensation lipid metabolism (children with satisfactory compensation of lipid metabolism (total cholesterol level less than 5.2 mmol / l), children with poor compensation lipid metabolism (total cholesterol above 5.2 mmol / l)). Nature of the subjects studied nutrition using  a questionnaire drawn up on the basis of the standard dietary questionnaire program CINDI, allowing to evaluate the eating habits by examining the frequency of intake of various foods, food preferences and priorities. Products, according to the lipid-lowering diet on the recommendations of the European Society for atherosclerosis were combined and divided into groups according to frequency of use: commonly used products, and rarely use the products. Mathematical treatment of the results was performed using the software package Statistica 5.5 for Windows. In the first phase expected to total group performance statistics – the arithmetic mean value M and the error of the mean m, as well as visualization of the distribution of values was performed using frequency histograms. To select a criterion to assess the significance of paired differences check compliance form normal distribution using the criterion χ2, and also controlled the general equality of variances using the F-test Fisher. Given that these conditions are satisfied only part of the empirical distributions of features tested the hypothesis that the general average in all cases was performed using Mann–Whitney U-test for independent variables. Null hypothesis was rejected when p < 0.05.

In the study of lipid metabolism in patients with type 1 diabetes, no increase in the level of cholesterol, triglycerides, HDL, LDL and VLDL – cholesterol in patients with a combination of diabetic nephropathy and retinopathy compared with patients with type 1 diabetes without diabetic microangiopathy. Investigation of the nature of power in patients with type 1 diabetes, allowed us to estimate its impact on lipid metabolism.

Abstract

Today the questions about possibility of development disturbances in the immune system of the fetus and the newborn in chronic toxoplasmosis are poorly understood. Aim of research: to detect immunological disturbances in the offspring of rats which have been administered antigens T. gondii.

Two series of experiments was performed. In these experiments white female Wistar rats in the III trimester of pregnancy have been administered corpuscular antigen T. gondii. The 60 days-old offspring of these rats have been included in study group of 137 animals. CD3+ cells count was performed in peripherical blood and standard suspension of splenocytesrats offspring. Peripherical blood cells count was performed in the blood of the rats offspring. In the second experiment rats offspring have been administered sheep erythrocytes in 5 days, before euthanasia. In spleen of this rats antigen-produced cells was counted.In control group was included 118 animals, which was born from white female Wistar rats have been administered 0,9% NaCl solution. CD3+ cells was detected in Cytomics FC500 flow cytometry analyzer (Beckman Coulter,USA) by use rats origne-specifed monoclonal antibodies Anti-Rat CD3-FITC (Beckman Coulter,USA). Hematological parameters was assessed by use hematological analyzer Excell-22 (USA).

We observed, that CD3+ lymphocytes and antigen-produced cells was decreased in test group (degress of decrease CD3+ cells was 17,2%; р = 0,003 in spleen vs. control group, degress of decrease antigen-produced cells was 27,3%; р = 0,03 vs. control group). Number of leukocytes was increased in in test group (34,5%; р = 0,009 vs. control group). Power and strength correlation pleiades between studied blood and spenal markers were higher in in test group vs. control group (∑Gi = 16; ∑Di = 4,38 vs. ∑Gi = 13; ∑Di = 2,28). This phenomenon is probably due to the development adaptive reactions disruption in the immune system and development secondary immunodeficiency states in the offspring of rats.

Violations of immunoreactivity, as well as disruption the possibility of operational restructuring of the immune system and choosing the best immune response in the offspring of rats whose perinatal development take place under the effect of antigens T. gondii, persists for a long time – within 60 days of postnatal life.

Abstract

The paper presents the results of assessing general and elastic lung hysteresis in 21 healthy volunteers and 28 patients with community-acquired pneumonia. Negative elastic hysteresis was detected in 14 healthy volunteers and 18 patients with community-acquired pneumonia. For the first time ever, it was demonstrated that, under the conditions of an interrupted air flow, 4 healthy volunteers and 2 patients with community-acquired pneumonia were characterized with the inversion of not only elastic but also general lung hysteresis. In case of pneumonia, despite the increase in tissue friction resulting from the generalized escalation of the alveolar-capillary permeability, the inversion of both elastic and general lung hysteresis takes place as often as in healthy people. It can be explained with the intensification of the mechanical activity of the lungs as a compensatory reaction in the mechanical system of the external respiration organs in case of pneumonia.

Abstract

The aim of the work was the study of connection of allelic polymorphism of IL10 and TGFВgenes with changes in the basal and BCG-induced production of immunosuppressive cytokines IL-10 and TGF-β by mononuclear leukocytes in vitro in patients with the first diagnosed pulmonary tuberculosis (TB), depending on the clinical form of the disease. The evaluation of the cytokines production was conducted by measuring its concentration in culture supernatants by ELISA. The allele-specific amplification of specific stretches of the genome was used for the study of polymorphic genes of cytokines. The DNA and supernatants of culture suspensions of blood mononuclear leucocytes in healthy volunteers and patients with TB were the material of the research. It was shown in the research conducted that the basal and BCG-induced over-production of IL-10 in vitro occurs in patients with TB, regardless of the genotype of the locus of C-592AIL10 gene. In addition, genotype AA of polymorphism of IL10gene in patients with infiltrative and disseminated TB is associated with the maximum production of IL-10 in vitroand genotype CC – with the minimum production of this cytokine in vitro. Analysis of the production of TGF-β in vitro in patients with TB showed its increase only in case of carriage of allele T (C-509T) of TGFB gene. In patients with disseminated TB and homosygotic genotype TT the increase in both basal and BCG-induced production of TGF-β was determined, and in patients with infiltrative TB – only after induction of cells by BCG-antigen.

Thus, the over-production of cytokines with inhibiting activity in patients with TB is genetically determined and promotes the formation of suppressive mode of immune-regulation. The increase in the secretion of cytokines IL-10 and TGF-β in vitro in patients with TB are associated with carriage of allele A and genotype AA (C-592A) of IL10gene and allele T and genotype TT (C-509T) of TGFB gene.

Abstract

The development of optimal experimental therapeutic schemes of immobilized interferon α-2b (production of JSC «Siberian Center for Pharmacology and Biotechnology», Novosibirsk) administration was conducted in the frame of the common program of pre-clinical study. The drug being studied was administered intragastrically for 3, 5 and 7 days in the therapeutic dose (1.8 × 105 IU/kg).  Experimental evaluation of immunotropic properties of immobilized interferon α-2b was carried out at in vivoexperiments in outbred CD1 male mice with using of following tests: the study of drug effect on the phagocytic activity of peritoneal leukocytes (the relative number of phagocytes and the number of bacteria which were assimilated by phagocytes were assessed); the study of drug effect on the humoral immune response (the total number of splenocytes, the relative and absolute number of antibody-forming cells and their functional activity were determined); estimation of drug effect on the cellular immune response with the using of delayed-type hypersensitivity test induced by sheep red blood cells. Based on these data we concluded that course administration of immobilized interferon α-2b in the therapeutic dose for 5 days increased phagocytosis of neutrophils and humoral immune response most effectively. Cell-mediated immune response was significantly increased after 7-fold administration of studied drug.

Abstract

Bronchopulmonary system diseases are on the first place among the causes of people's death. Most of methods for lung diseases diagnosis are invasive or not suitable for children and patients with severe disease. One of the promising methods of clinical diagnosis and disease activity monitoring of bronchopulmonary system is analyzing of human breath. Directly exhaled breath or exhaled breath condensate are using for human breaths analyzing. Analysis of human breath can apply for diagnostic, long monitoring and evaluation of efficacy of the treatment bronchopulmonary diseases. Differential diagnostic between chronic obstructive lung disease (COPD) and bronchial asthma is complicated because they have differences in pathogenesis. Analysis of human breath allows to explore features of COPD and bronchial asthma and to improve differential diagnostic of these diseases. Human breaths analyzing can apply for diagnostic dangerous diseases, such as tuberculosis, lung cancer. The analysis of breath air by spectroscopy methods is new noninvasive way for diagnosis of bronchopulmonary diseases.

Abstract
The review covers the current concepts on structural and functional heterogeneity of brain astrocytes that serve for numerous (patho)physiological processes in the central nervous system. Astrocytes from various subpopulations demonstrate different sensitivity to the action of pathogenic factors, varied behaviors in reactive processes and within the local immune response. Key functions of astrocytes like neurogenesis, neuron-astroglia metabolic coupling, glial control of local blood flow greatly depend on the origin and characteristics of astroglial cells. Abstract

On the basis of phylogenetic theory of general pathology, the cause of a noninfectious disease whose occurrence in a population is more than 5–7% is an impaired biological function or reaction to the environment. From the general biology viewpoint, high mortality rate related to cardio-vascular diseases and atherosclerosis (intercellular deficiency of polyenic fatty acids (PFA)) is just extinction of the Homo sapiens population upon adaptation to new environmental factors. The biological function of throphology (feeding) and biological reaction of exotrophy (external feeding) are impaired in several aspects, the major of which is nonphysiologically high dietary content of saturated fatty acids, primarily, of palmitic fatty acid (FA). The lipoprotein system formed at early stages of phylogenesis cannot transport and provide physiological deposition of great amounts of palmitic FA, which leads to the development of an adaption (compensatory) and accumulation disease. This results in hypermipidemia, impaired bioavailability of PFA to cells, compesatory production of humoral mediators from ω-9 eicosatrienoic mead FA, disorders in physiological parameters of cell plasma membrane and integral proteins, nonphysiological conformation of apoВ-100 in lipoproteins, formation of ligandless lipoproteins (biological litter) and impairments in the biological function of endoecology, utilization of ligandless lipoproteins in arterial intima by phylogenetically early macrophages that do not hydrolyze polyenic cholesterol esters, increase in the intensity of the biological reaction of inflammation, and destructive and inflammatory lesions in arterial intima of an atheromatosis or atherothrombosis type. Atheromatous masses are catabolites of PFA which were not internalized by phylogenetically late cells via receptor-mediated pathway.

Abstract

Presented modern literature data on the features of the pathogenesis, course, clinical and morphological expression and tumor characteristics, parameters and nodal metastasis of hematogenous bilateral breast cancer. Highlight the results of domestic and foreign studies in recent years to determine the prognostic factors and recurrence of synchronous and metachronous bilateral breast cancer. It was revealed that the frequency of bilateral breast tumor lesions varies widely, ranging from 0.1 to 20%, with metachronous tumors recorded significantly higher (69.6% ) than the synchronous (22.7%). The probability of occurrence of metachronous breast cancer is higher in women with a family history, as well as if they have a gene mutation BRCA-1. Found that the most common histological type of breast tumor with bilateral lesions is invasive ductal. However, the incidence of invasive lobular cancer and non-invasive lobular cancer is slightly higher among synchronous bilateral cancer compared with unilateral disease. Studies have shown that in a double-sided synchronous breast cancer tumor, as a rule, has a lower degree of differentiation, and the higher the expression level of estrogen receptors and progesterone receptors. Relevance of the issue because the identification of patterns in the study of lymphatic and hematogenous features bilateral metastasis of mammary tumors provides a basis for speculation about the differences in the progression of neoplastic disease in these groups and is a cause for further detailed research in this area to identify and evaluate the prognosis and also the choice of tactics of such patients.

Abstract

Endothelial dysfunction is an early marker for the development and progression of cardiovascular diseases. Scientific studies in recent years have shown the necessity to study the endothelial function in different groups of patients in clinical practice. This article is focused on the possibilities and perspectives for clinical use of laboratory and instrumental methods for the study of endothelial function. One of the factors causing the development of vascular disease in men is testosterone deficiency. The review highlights the most important mechanisms of action of sex hormones on the vascular endothelium and its function in men. The data about the effects of hormone replacement therapy with testosterone on endothelial function in patients with late onset hypogonadism were critically analyzed.

Abstract

Analysis of the current literature showed that in publications, in our opinion, there is no master: a scientific analysis of the reasons behind the decline in the quality of knowledge of the doctor, as well as the scientific basis of modern pedagogical technologies introduced in the process of training future doctors to address this problem. Objective and subjective factors are having a negative effect on the quality of the doctor's knowledge. The generally accepted methods of assessing the quality of knowledge – testing and rating system – have their limitations. Testing reflects the level of knowledge about the thinking style of the originator of test tasks. Ranking system for assessing knowledge increasingly characterized by diligence in studying medicine. It is proposed to upgrade the quality of medical knowledge in the teaching of the theory of integration of diagnostics. Amount of knowledge to evaluate the content of educational narrative history of the disease. The quality of the knowledge to assess the content of professional clinical picture comment in the history of the disease.

Abstract

The paper reviews topical issues of organization and management of innovative activity in the regional health care system.

Objective. Development and scientific substantiation of a conceptual model of managing innovation in the regional health care system, introduction of institutional mechanisms for its implementation, and evaluation of their efficacy in using diagnosis and treatment technologies. Objectives of the study included reviewing the organization status and problems hampering the development, identification of prospects, and justification for appropriate changes in innovation in healthcare system and medical science at the regional level.

Material and methods. To conduct a comprehensive assessment of the status and meet challenges of innovation promotion, a methodology for social-hygienic research was worked out including bibliographic and analytical methods, situational analysis, sociological and economic methods, expert assessment, methods for quality management system audit in accordance with ISO 19011:2002, IDEFO function modeling (RD IDEF0-2000), and organizational modeling. The study was based on the analysis of foreign and domestic literature, statistics, methods for managerial modeling, as well as management experience (including innovative methodological approaches) gained by Novosibirsk Research Institute of Traumatology and Orthopedics named after Ya.L. Tsivyan which meets the requirements for a platform for research and innovative product reproduction, including research, clinical, organizational, and managerial aspects. Other facilities were considered in conjunction with the leading innovative platform.

Results. The paper presents a scientifically based model of innovative medical environment with its elements as subjects, each with a set of functions. Conceptual model for management includes structuring (resource, processes, and quality management; application of international standards) and strategic management mechanisms, provides a systematic approach to management, and ensures efficient operation of medical institutions and public access to innovative medical diagnostic technologies. The advantage of innovative products and technologies using within the doctrine of innovative management is that they provide a comprehensive approach to the diagnosis and treatment of patients. The introducted managing innovation technologies allow to improve organizational performance of healthcare institutions and to achieve significant medical and social effects: high level of diagnosis and treatment, improved access and quality of care what promotes the public health.

Conclusion. Formation of regional innovation medical environment with advanced innovation infrastructure, interaction of its elements through an effective system of coordination and control is a prerequisite and basis for more mature and system-level structure – an industry innovative cluster. Active development of innovation in health care based on cooperation among the different actors of innovation and interagency collaboration provides a professional staff to the sphere of research commercialization, accomplishes innovative technology transfer to a real economy, and enables the integration in the worldwide innovation environment through the implementation of technological and scientific research projects.

The aim of the study was to identify the key problems of regional health services in the Siberian Federal district and justification of the priority directions of their decision. For the first time using the method of expert estimations conducted a study of health problems in the Federal district, identified key issues, as well as justifies the priority directions of their decisions. The study was conducted on the basis of statistical materials of the health authorities of the regions of Siberian Federal district, as well as expert assessments. Abstract

20 men (medium age (31.5 ± 13.5) years) were studied. All patients underwent arthroscopic auditing plastic anterior cruciate ligament of the knee. Osteoregeneration dynamics were determined by biochemical testing of blood serum and X-ray computed tomography (CT) of the fracture in 1–4 months after surgery. As a result, an estimation of the bone tissue reparative regeneration according to CT data in 10 patients with a defect of the patella, treated with the material is provided. The obtained results of the clinical study indicate the reparative effect of the biodegradable material for bone replacing on bone regeneration
